Floor, Gali No.2, Dayanand Block, Shakarpur, Delhi – 110092, do solemnly
state and affirm the following: -

1. That I am a citizen of India.

2. That my ward Ashish Kumar S/o Late Sh. Lalji Prasad is a student of
Class 11th Commerce in ANDHRA EDUCATION SOCIETY
SCHOOL, ITO.
3. That I am the mother and sole natural guardian of my ward.
4. That I belong to General Category and does not belong to any of the
SC/ST/OBC category.
5. That my annual Family income from all sources amounts to Rs.
1,20,000/- (One Lakh Twenty Thousand Rupees Only).
