The requirement is for the EVALUATION REPORT FOR COLLECTION OF REVENUE FROM
LOCAL SERVICE TAX SOURCE. LIRA758/SRVCS/2019-2020/00002
The Procurement method is Open Domestic Bidding.
Details of Invitation
The bid notice was issued on 25th June, 2019. The list of bidders purchasing the Bidding
Documents was recorded using LG PP Form 6: Record of Issue of Solicitation Documents, Fee

100,000/= which is attached as an annex. .
Details of Bid Closing
Bidding was closed on 22nd July, 2019 at 10:00am at PDU. The receipt of bids was recorded
using LG PP Form 9: Receipt of Bids, which is attached as an annex. Sealed Quotations were
required and received.
Details of Bid Opening
A Public bid opening was held at LMC Health board room on the 22nd July, 2019 at 02:00pm.
The bid opening and attendance was recorded using LG PP Form 10, which is attached as an
annex.
Details of Evaluation Committee
An evaluation committee was nominated comprising the following officials and approved by the
Contracts Committee on 15th July, 2019.
