TRANSFERRING A PATIENT FROM WHEELCHAIR

1. Firstly, I will try to look at the situation as a whole. If there is need, I would ask
for help to another staff member because transfer must be conducted safely,
with dignity.
2. Patient safety is often the main concern when moving a patient from
wheelchair.
. I will check the patient need one assistance or more because transfer often
requires the patients help. Due to this patient must be co-operative and able
to
bear weight on both legs and take small steps. If any of these criteria not met,
a two-person transfer is recommended.
 I will check room for additional precautions.
 Introduce yourself to patient.
 Confirm patient ID (e.g., name and date of birth)
 Listen and attend to patient cues.
 Ensure patient safety and privacy. 

3. RULES: -
 Perform hand hygiene.
 Must lock the brakes on wheelchair and the bed. While in the sitting
position the patient’s feet should be flat on the floor.
 Wheelchair place near to the bed.
 Explain the patient what will happen and how patient can help.
 Use correct form. It is important to protect lower back of the patient
when transfer him.
 Move the person safely. Do not pull the patient by the arms.
 Ask the patient to help as much as possible. also ask someone to help
move the patient if needed.
 After the transfer, make sure the person is comfortable.
 Check all medical equipment and tubes that needs to be adjusted after
the move.
