Situation 1

Nurse : “Hello, good morning ma'am, can I help you?”

Patient : "Morning nurse, I want to meet doctor dodi, is doctor dodi on duty
today?"

Nurse: "Yes, I have told doctor Dodi earlier. because someone wants to meet
you."

Patient: "Sure, nurse"

Nurse : "Okay ma’am, we can fill out the patient form beforehand."

Patient : Okay nurse

Nurse: "What's your name?"

Patient : “My name is Minah siti rosita dewi”

Nurse: "What is your date of birth?"

Patient : “on July 6, 2001”

Nurse: "What is your job?"

Patient: "entrepreneur"

Nurse : "May we know your marital status?"

Patient: "I'm already married"

Nurse: "Who can we call the next of your kind?"

Patient : “You can call my husband, this number is 08217829”

Nurse: "Do you have a smoking intake?"

Patient: "No, never."

Nurse: "Do you have any alcohol intake?"

Patient: "No, never."

Nurse: "What's the reason you came in?"

Patient: "Because, I have a problem with pain I have a problem with

headaches and my neck feels heavy."

Nurse: "May I know your medical history?"

Patient: "I don't have any hospital charts."

Nurse: "What about your family history?"

Patient: "My father has a history of high blood pressure."

Nurse: "Okay, ma'am, to find out more about the complaints that you are
experiencing. We will follow up the examination on Mother and Mother must
be hospitalized.”
Situation 2

Nurse : Good morning, ma’am

Patient : Good morning, nurse

Nurse : Okey ma’am, my name is Aira Nazais Prameswari. You can call me
nurse aira, and what is your name?

Patient : My name is Ma’am mina

Nurse : Ma’am. Mina , i will take care of you from this morning until
afternoon. Exactly at 2 pm. How are you today, ma’am?

Patient : There is something wrong with me, i have a problem with headache
and my nape feel heavy.

Nurse : Did you sleep well last night ma’am?

Patient : Not really, cause i still got a problem with my headache.

Nurse : Okey ma’am. Mina , this morning i will check your condition, i would
like to measure your blood pressure. May i measure your blood pressure?

Patient : Surely nurse aira , i’m ready, please check it.

Nurse : Would you mind to stretching your hand, please?

Patient : Of course

Nurse : Excuse me, ma’am. Would you mind roll your sleeve up?

Patient : Sure
Nurse : (Checking the blood presurre)

I have finish measured your blood pressure

Patient : What is my blood pressure, nurse?

Nurse : Your blood pressure is 150/90 mmHg. You got a problem with your
blood pressure cause is too high range and i will report this to a doctor.

Patient : Okey nurse, thankyou for your information, i’ll change to my lifetyle.

Nurse : Your welcome and after that you should take your medicine, be
healthy ma’am

Patient : Yes nurse

Nurse : Ok ma’am if you need something, you can call me in nurse station

Patient : Yes, thank you nurse.

Nurse : Your welcome, Ma’am.  Get well soon
