Note:

The foregoing is the format for a notarial will. The will may likewise be drafted
in the form of a holographic will. In a holographic will, the Attestation Clause
and the Joint Acknowledgment are not necessary. In addition, the following are
the other requirements for a holographic will, as provided by Article 810 of the
Civil Code:

a) The will must be entirely hand-written by the testator / testatrix;

b) The will must be dated in the testator’s / testatrix’s handwriting;
c) The will must be signed by the testator / testatrix.
