and currently the Supervisor in our Facility department.

Pollution Control Officer.

Attached are the following requirements:

1) PCO Accreditation Application Form (either from NCR or RIV-A)

2) Letter of appointment / designation as PCO by Managing Head
3) Certificate of Employment
4) Curriculum vitae with 2x2 ID picture (2 pcs.) of the appointed/designated PCO
5) Transcript of Records and Diploma
6) Notarized Affidavit of Joint Undertaking of the PCO and the Managing Head
7) Proof of the appointee’s qualification such as Certificate of Training of Basic PCO
Training
8) For Category B Establishments, include the valid Professional Regulation Commission
license, if applicable.
9) Managing Head Certificate of Seminar/Orientation for 8 Hrs.
