OFFER OF REGULAR EMPLOYMENT

The Company is pleased to advised you that, following an evaluation of your performance, you
are offered Regular Employment in the position of ___________________, under the following terms
and conditions:

1. DATE OF REGULAR EMPLOYMENT


For the purpose, your date of regular employment with this Company shall be
___________________.

2. SALARY
Your salary shall be Pesos____________________________ (P_____) per month. This
compensation shall be payable semi-monthly, and shall be in cash or in other methods as
may be implemented by the Company. Applicable withholding taxes, SSS, Philhealth,
PAGIBIG, and other similar government–mandated deduction shall be made on your
payroll. Accountabilities for money and Company property shall likewise be deducted from
your salary in case of your failure to account or return the same to the Company. Increase in
salary & giving of allowances is solely at the discretion of the Company.

3. HOURS OF WORK
Your actual and paid work schedule is eight (8) hours a day, six (6) days a week. You are
expected to perform shift duties during your working hours and to render overtime work
when required by the company in cases specified in Article in 89 of the Philippine Labor
Code. Any overtime work will not be paid unless prior written approval is obtained from the
authorized management

4. RULES AND REGULATIONS


You shall at all times abide by the Company’s Rules and Regulations which shall be notified
by you by office staffs and it will be your obligation to read all memorandums posted at the
bulletin board which is located at the entrance door of the office. You are expected to serve
the Company diligently, efficiently and to the best of your ability and to the Company’s
satisfaction.

5. PUBLIC HOLIDAYS/ROSTER DAYS OFF


You are expected to work when required during public holidays and/or roosted days off days
for which you shall be compensated according to the provisions of the Labor Code but it
shall be the prerogative of the Company to reschedule your entitlement days on other days
according to needs and demands of the Company.

6. TRANSFERABILITY
The Company shall also be entitled to assign, second and transfer you to any subsidiary of
the Company.

7. ASSIGNABILITY & DESIGNATION


The Company shall be entitled to change your designation, area of assignment, working
hours, job responsibilities and such other work related matters to meet or suit the needs and
demands of the Company.

8. RESIGNATION/TERMINATION
In the event of your resignation, you shall be required to give thirty (30) days written notice
prior to the effectivity of your resignation.

9. CONFIDENTIALLY
You shall treat all Company’s records, documents and all information pertaining to the
business of the Company as strictly confidential and that no unauthorized disclosure or
reproduction of the same will be made by you at any time during or after your employment
with the Company. You voluntarily agree not to divulge, during or after your employment,
any trade secret of the Company, including but not limited, to the identity of customers,
volume of production and sales, design secret, techniques and know how and other
information of a similar nature that may come to your knowledge during your employment
with the Company.

10. DOCUMENTS AND PROPERTIES


You are to surrender immediately all Company’s record, documents and properties in your
custody and control, upon request at any time during the period of your employment, and
immediately at termination thereof, whether or not requested.

11. OTHER EMPLOYMENT


You shall not any time during employment with the Company, engage and/or accept any
other employment with any other party without the express written consent of the Company.

12. SOLE AGREEMENT


You voluntarily agree and understand that there are no other agreements or understanding
between you and the Company or any of its officers, representatives or agents. Concerning
or affecting your employment with the company.

You further voluntarily agree that no amendments, modifications or alternations of the terms
and conditions of this Regular Employment shall be binding upon either yourself or the
Company unless the same is reduced in writing and duly consented to, approved and
signed by you and the company.

If the above terms and conditions are accepted to you, kindly acknowledge the same by signing of this
letter.
