OFFER OF CASUAL EMPLOYMENT

Further to your application and a recent interview, (COMPANY) is


pleased to offer you Casual Employment for the position of
_____________ subject to the following terms and conditions:

1. PERIOD OF EMPLOYMENT

Your employment with the Company shall start on ________ and


terminate on ____________.
2. COMPENSATION

During your tenure, you shall receive by way of compensation the


amount of ________ a day. This compensation shall be payable semi-
monthly in cash. SSS deduction shall likewise be made on your wages.
Accountabilities for money and Company property shall likewise be
deducted from your salary in case of your failure to account or return the
same to the Company.

3. DUTIES AND RESPONSIBILITIES

In your designation, you will be assigned as Personal Driver You will


report to the office for any matter relating to your conditions of
employment.

4. MEDICAL FITNESS

The offer of Casual Employment is conditional on you being certified


medically fit and free of any or all contagious diseases by the Company-
appointed medical practitioner.

5. HOURS OF WORK

Your actual and paid work schedule is eight (8) hours days a week. You
are expected to perform shift duties during your working hours and to
render overtime work when required by the Company. You will be
compensated according to the provisions of the Philippine Labor Code.
Any overtime work will not be paid unless prior written approval is
obtained from the authorized management.
6. RULES AND REGULATIONS

You shall at all times abide by the Company’s Rules and Regulations
which shall be notified by you by office staffs and it will be your
obligation to read all memorandums posted at the bulletin board which is
located at the entrance door of the office. You are expected to serve the
Company diligently, efficiently and to the best of your ability and to the
Company’s satisfaction.

7. CHANGE OF DESIGNATION

The Company shall be entitled to change your work roster, work hours
and such other work-related matters to meet or suit the needs of the
Company.

8. ASSIGNABILITY

The Company shall also be entitled to assign, second and transfer you
to any subsidiary of the Company.

9. CONFIDENTIALLY

You shall treat all Company’s records, documents and all information
pertaining to the business of the Company as strictly confidential and
that no unauthorized disclosure or reproduction of the same will be
made by you at any time during or after your employment with the
Company

10. DOCUMENTS AND PROPERTIES

You are to surrender immediately all Company’s record, documents and


properties in your custody and control, upon request at any time during
the period of your employment, and immediately at the termination
thereof, whether or not requested.

11. TERMINATION/TERMINATION

You may also have the option to voluntarily leave the employment of the
Company provided a notice of intention is filed thirty (30) days prior to
the effectivity of resignation.
12. SOLE AGREEMENT

You voluntarily agree and understand that there are no other


agreements or understanding between you and the Company or any of
its officers, representatives or agents. Concerning or affecting your
employment with the Company.

You further voluntarily agree that no amendments, modifications or


alterations of the terms and conditions of this Casual Employment shall
be binding upon either yourself or the Company unless the same is
reduced in writing and duly consented to, approved and signed by you
and the Company.

If the above terms and conditions are acceptable to you, kindly


acknowledge the same by signing of this letter.
