TO WHOM IT MAY CONCERN;

This is to certify that VENCIA MAYO LUYAD, 46, a resident of PUROK

GUMAMELA, PULA-BATO, TAMPAKAN, SOUTH COTABATO has died due to
heart attack this 15th of MAY, 2021 at the residence of BERNABE B. FRANCO of
Sitio 7, Vizal sto. Cristo, Candaba, Pampanga. And as per report of the Barangay
Health Emergency Response Team (BHERT) that he/she is not in the list of persons
who are suspected, nor active case as regard to COVID-19 pandemic.
This is to certify further that the family of VENCIA MAYO LUYAD at South
Cotabato requested the transfer of the body in their province.

This certification is issued upon the request of the above named individual for
whatever purpose it may serve.

Given this 17TH day of MAY, 2021 at Barangay Vizal Sto. Cristo, Candaba,
Pampanga.
