Evacuation Center
This agreement is made and entered into between VIZAL STO. CRISTO ELEMENTARY
SCHOOL and Barangay COUNCIL OF VIZAL STO. CRISTO to establish
shelter/Evacuation center site locations and terms of use in the event of an evacuation for
the residents of BARANGAY VIZAL STO. CRISTO, CANDABA, PAMPANGA.

Whereas the school Principal/Administration of VIZAL STO. CRISTO

ELEMENTARY SCHOOL will make every effort to notify Barangay OFFICIALS OF VIZAL
STO. CRISTO of evacuation possibilities with as much notice as possible. Contact
information between the two parties shall be maintained considered confidential
information and is not subject public disclosure unless agreed by both parties;

Whereas VIZAL STO. CRISTO ELEMENTARY SCHOOL agrees to open their

building located at SITIO 2 VIZAL STO. CRISTO, CANDABA, PAMPANGA to provide
shelter and assistance to residents evacuated during emergency situations when need to
be sheltered.

Whereas VIZAL STO. CRISTO ELEMENTARY SCHOOL has a capacity to

accommodate approximately 600 people.

Whereas VIZAL STO. CRISTO ELEMENTARY SCHOOL understand that their

School/Organization/Building will be responsible for opening the building and developing
procedures for making the building accessible, including rest rooms and an area with
phone and internet connection (if available) for Barangay Officials of VIZAL STO. CRISTO
and administrative personnel. Furthermore, the head of the
school/Organization/Building/Barangay Officials will provide supervision for all the
residents during the time that the facility is used as an emergency shelter site.

Whereas Administrator/Barangay Officials agrees that it shall exercise reasonable

care in the conduct of its activities in said facilities and further agrees to replace or
reimburse VIZAL STO. CRISTO ELEMENTARY SCHOOL for any items, materials,
equipment or supplies that may be used for evacuation or facilities.

Whereas Barangay Officials will be responsible for replacing, restoring or

repairing damage occasioned by the use of any building, facilities or equipment belonging
to VIZAL STO. CRISTO ELEMENTARY SCHOOL

Nothing in this MOU is intended to conflict with current laws or regulations of the
Republic of the Philippines or local government. If a term of this agreement is inconsistent
with such authorit, then that term shall be invalid, but the remaining terms and conditions
of this MOU shall remain in full force and effect.

This agreement shall become effective on 16th day of March, 20 19 and may be modified
upon the mutual written consent of the parties.

The term of this agreement, as modified with the consent of both parties, shall be self-
renewable for a period of five (5) years from the end date of the agreement unless written
termination is given by either party. Either party, upon sixty (60) days written notice to the
party, may terminatethis agreement.
The terms of this agreement, as modified with the consent of both parties, AND NOW, this
16th day of March 20 19, the parties hereby acknowledge the foregoing as the terms and
conditions of their understanding.
