Corrective maintenance performed on fire hoses in fire


hose cabinets as evidenced by the closeout of previously
issued finding under reference no. PMO-VWR-NCR-07/17
Yukon Technical (Series no. 8182) dated May 172017 proved to be
Maintenance Corrective Maintenance ineffective. After 3 months since the finding was closed on
July 252017 re-positioning of fire hose in third quarter of
2017 in Aqua Canary Flamingo Peridot and Wisteria
Building were not performed by the Technical
Maintenance as of audit date.

Rich-Free Trading and Devt Corp. has ten (10) year


warranty for waterproofing works against material defects
in elevator pit. However after two (2) years from
RICH FREE acceptance date failure in waterproofing works on
TRADING &
DEVELOPMENT Predictive Maintenance elevator pit in Peridot Building was observed at time of
audit that caused water ponding. Furthermore incident
CORP. report was not prepared by the PMO to address the issue
and portable sump pump has not yet provided to
eliminate the water ponding.

Fire Safety Inspection Certificate (FSIC) in Verawood


PMO Preventive Maintenance Residences is valid until April 2018.

Though the potability test was expired on October 22


PMO Preventive Maintenance 2017 PMO processed the payment for renewal and the
potability test is currently on-going at time of audit.

There was a settlement of drop-off area and stair (pebble


wash-out in between) at main entrance of Wisteria
building of almost 25 mm. Though PMO provide a
Yukon Technical summary report of punchlist on February 7 2016 but it
Maintenance Corrective Maintenance was sent to the previous Project-In-Charge of Verawood
Residences (Renier Mendoza). Settlement of drop-off area
in Wisteria Building was not relayed to the current Project-
In-Charge (Lemuel Sacan) to provide necessary action to
address the issue.

The preventive maintenance of lightning arrester was not


performed by the maintenance personnel to wit;a. visual
Inspection;b. complete testing and inspection including
Yukon Technical Preventive Maintenance the following:- test to verify continuity of those parts-
Maintenance
ground resistance test- continuity test- inspection of all
conductors and system components- tightening of all
clamps and splicers

Maintenance of cistern tank was inadequately performed


Yukon Technical by the Technical Maintenance. As a result manifestation of
Corrective Maintenance corrosion and detached metal cover of cistern tank were
Maintenance observed and the preventive maintenance were not
presented at time of audit.

Mandatory Requirement Sanitary Permit to Operate is valid until December 31 2019


PMO Compliance as of audit.

PMO Mandatory Requirement Business Permit is valid until December 31 2019.

Inadequacy of monitoring and checking on sprinkler


system by the Technical Maintenance. As a result
manifestation of corrosion were observed at the
escutcheon plate on the 6th floor and upper ground floor
Yukon Technical Corrective Maintenance of Wisteria Building upper ground floor and 2nd floor of
Maintenance Marsala Building and upper ground floor of Peridot
Building. Further incident report to address the issue was
unavailable on the project site and has no PM checklist as
proof of regular maintenance as of audit date.

The provision of the DPMC Work plan in maintenance of


generator set was sufficiently performed by the Technical
Maintenance. Generator is in good running condition
Yukon Technical Preventive Maintenance weekly housekeeping of genset was properly performed
Maintenance and free from manifestation of corrosion as of audit date.
Documentation of latest conducted preventive
maintenance was also presented by the Facilities Officer
during the audit.

Monthly inspection of main distribution panel was already


included in the DPMC Work plan however the technical
Yukon Technical maintenance was not complied it provision. As a result the
Corrective Maintenance following findings were obeserved in Flamingo and Lime
Maintenance building at the time of audit:a. power meter of the MDP
have no display or reading; andb. have no maintenance
report or any documents as proof of regular monitoring.

The provision of the DPMC Work plan in maintenance of


electric fence was sufficiently performed by the Technical
Yukon Technical Preventive Maintenance Maintenance. Electric fence is in operation and free from
Maintenance any trouble or fault as of audit date. Documentation of
latest conducted preventive maintenance was also
presented by the Facilities Officer during the audit.

The provision of the DPMC Work plan in maintenance of

Preventive maintenance performed on the lift station and


sewage pumps were proved to be effective. Lift station
WILSERV MULTI- and sewage pumps were in good operational condition

as proof of regular checking and monitoring dated
October 1 2019.
Monitoring checking and cleaning on the sump pit were
adequately performed by the technical maintenance.
Yukon Technical Sump pumps were all in good operating condition and no
Maintenance Preventive Maintenance debris or garbage on the sump pit and preventive
maintenance checklist for the month of September 2019
was available on project site at the time of audit.

Yukon Technical Quarterly re-positioning of fire hoses indicated in the


Maintenance Preventive Maintenance DPMC Work plan was performed and complied by the
technical maintenance for the 4th quarter of 2019.

Field Master Fire department connections at Buildings Wisteria to Lime


Manpower Preventive Maintenance were fully visible with tagging and recognizable from the
Services street or nearest point of fire department apparatus.

Yukon Technical performed by the Technical Maintenance were proved to
