I, _____, legal age, single, and a resident of Cebu City after having
sworn in accordance with law, do hereby depose and state that:

1. I am one of the passers of the Bar Examination and my name appears

in the official list released by the Supreme Court on _____;

2. I attended the oath-taking ceremony via online video conference last

____ while I was within the Philippine territory, particularly in the
City/Municipality ____;

3. During the ceremony, I took or had another person take the attached
photography or screenshot of the video conference showing both
myself and the en banc proceedings (Annex A);

4. I have paid the bar admission and certification fees amounting to

P3,850.00 via postal money order, and have attached herewith the
proof of payment (Annex B);

5. I have duly executed and have herewith attached the prescribed

Alternative Roll Signing Form containing my right thumb mark print,
official signature, and a copy of my valid government-issued
identification card showing my official signature (Annex C) for the
sole purpose of complying with the alternative roll signing procedure
as per Supreme Court Resolution dated ____, and for the Office of the
Bar Confidant’s (OBC) official records;

6. The OBC may send the Certificate of Membership to the Philippine

Bar, bar rating, and official receipt to me through this email address:

7. I undertake to sign the Roll of Attorneys and register my biometric

information before the OBC at its actual office located in the Supreme
Court building, Padre Faura Street, Ermita, Manila on or before
December 31, 2020, unless further extended by the OBC due to public
health considerations;

8. I acknowledge that my failure to comply with the aforesaid

undertaking within the prescribed period shall result in, among others,
the immediate imposition of the appropriate administrative sanctions
affecting my good standing as a member of the Philippine Bar.

 9. I am executing this affidavit to attest to the truthfulness of the above-
stated facts under pain of contempt and the appropriate penalties and
sanctions pursuant to law.
