DETAILS OF COMPLAINT

I, MARITES LAZO ASUNCION, a legal age of 22 years old, a Filipino

citizen from 19-E Road 4 Brgy. 1 st West Crame, San Juan City, Metro
Manila. That OOA 8:30 PM of September 8, 2021 at 19-E Road 4 Brgy.
1st West Crame, San Juan City, Metro Manila. I was driving my
motorcycle with a plate number of KC 80999 along the road of Brgy. 1 st
West Crame, San Juan City near the intersection when I spotted two
policemen standing beside our barangay’s dental clinic. I turned my
tricycle to the right and suddenly Patrolman Trinidad called my attention
and directed me to stop by in front of the barangay post. Patrolman
Trinidad approached me and told me that the road during that time is a
ONE-WAY only. Patrolman Quiado ordered me to show them my driver’s
license and OR/CR of the motorcycle. I gave them the papers and found
out that my license was already expired and not renewed. Patrolman
Quiado gave me a TCT of Counterflowing that fines Php 2,000 (Two
Thousand Pesos Only) and expired driver’s license that cost up to Php
3,000 (Three Thousand Pesos Only) and told me that I can get away
with that violation by just paying them Php 7,000 (Seven Thousand
Pesos). As soon as I gave them the money, I immediately went to the
nearest police station, here at PNP National Headquarters, Quezon City
and reported the incident about the latter.

that the submitted supporting documents are from verifiable source; and
that I am aware that I can be held liable for perjury and/or dishonesty as
a result of any fraud, false or misrepresentation in this affidavit. I
