PIPE WERE NOT TIGHTENED PROPERLY THAT CAUSE MINOR OIL SPILL AFTER PUT INTO SERVICE.
30 NOT FULLY TIGHTENED FLANGE BOLTS AND NUTS ON THE INLET LINE FOR M/E F.W. PRE-HEATER
