AFFIDAVIT OF BEING SINGLE 

I, JUANITO A. LAO-WAG, JR., Filipino, of legal age, single,

residing at Ambato, Legleg, Tinglayan Kalinga, after having been
sworn in accordance with law hereby depose and state:
 
1. That I am applying in the Military Service as
trainee in the Philippine Army;

2. That I was born on December 3, 2000 and one

of the children of spouses JUANITO N. LAO-
WAG and MARY A. LAO-WAG; 

3. That I am still single, as I have never

contracted any marriage with another person up
to the present;

4. That I am executing this affidavit to attest to

the truth of the foregoing and for whatever legal
purposes it may serve.
