AFFIDAVIT OF LOSS

             I, JONARD P. CALAGUI, Filipino citizen, of legal age, and currently residing at

Pinucoc, Pinukpuk, Kalinga, after having been duly sworn in accordance with law,
hereby depose and say:
 
1. That I was issued a passbook for my regular savings account by the Armed
Forces & Police Savings And Loan Association, Inc. (AFPSLAI) , and by
the Philippine Army Finance Center Producers Integrated Cooperative
(PAFCPIC) of the same;

2. That I placed the said two passbook in my bag together with my other
important documents and safely kept the same in our house at Pinukpuk,
Kalinga;

3. That sometimes on May 2021, when I looked for my passbook, said two
passbook were missing and nowhere to be found;

4. That I can no longer locate my two Passbook;

5. That I exerted diligent efforts to find and locate the same but to no avail;

5. That the said passbook was lost in the manner stated above and not
confiscated not canceled by the issuing agency or any instrumentality of the
government;

6. That I am now executing this this affidavit to attest the truth of the foregoing
narration and for all legal intents it may serve best.
  
 
            IN WITNESS WHEREOF, I have hereunto affixed my signature this 8th day of
July 2021 at Bulanao, Tabuk City Philippines.
