Handbook Receipt and Acceptance

I now acknowledge that I have received a copy of CrewBloom’s Contractor Handbook. I

understand that it is my responsibility to read the information contained in the Handbook. I
understand that the purpose of this Handbook is to provide me with a general overview of the
company’s policies and procedures. I acknowledge that nothing in this Handbook is to be
interpreted as employment at CrewBloom, expressed or implied, nor does it guarantee my contract
at CrewBloom for any period. I understand and accept that my contract with CrewBloom is at-will.
I have the right to end my contract with or without cause, just as CrewBloom may terminate my
contract at any time with or without cause or notice, both subject to applicable laws. I understand
that nothing in the Handbook or any oral or written statement alters the at-will relationship, except
by a written agreement signed by the Contractor and CrewBloom.

I acknowledge that CrewBloom may revise, suspend, revoke, terminate, change or remove,
prospectively or retroactively, any of the policies or procedures outlined in this Handbook or
elsewhere, in whole or in part, with or without notice at any time, at CrewBloom’s sole discretion.

By signing below, I acknowledge that I have fully read and understood everything written in
CrewBloom’s Handbook. I understand that if I have any questions or concerns about the Handbook,
I'm responsible for discussing this with CrewBloom’s Management Team.
