Learning Insights~

On the day of November 4, 2021 was the start of the 1 st day of performing the health
assessment. In the instructions there were 3 categories of patients that we were tasked assess
which were pregnant woman, well-baby and pre-school. The patient that I chose was a
preschooler because I knew someone who had a child which was still 5 years old. The objective
of this was to recognize the status of the preschooler physically, mentally and socially and to
recognize any signs that may indicate otherwise. Before we assessed the child, exactly about 2
days before November 4, primarily we had to call the child’s parent for consent and approval
which they eagerly agreed on.

Exactly on the day of the assessment, before going to the aforementioned patient, I still
had to prepare the necessary equipment and supplies needed for the assessment. Upon
entering their property, I was greeted by the patient’s mother who was very welcoming and she
introduced 2 of her children which both were still preschoolers but told us that the one we would
be assessing was the eldest which was the 5 years old. After introducing myself and showing
the mother and the patient what we were going to do, we went inside their house and did the
assessment. The assessment consisted of 7 parts which were the Demographic Data, Vital
Signs, Developmental Milestones, Immunization Record, Health Pattern Assessment,
Medications, and lastly the health assessment. Upon conducting the different assessments both
the mother and the child were very cooperative in everything that we did. For the mother she
was very cooperative and open to me and she was very supportive of her child by helping me
also in guiding the child and always staying by her side. For the child, she was shy at first but
still cooperated and had fun especially at the test of range of motion. Lastly for the husband, he
wasn’t there because November 4 was on weekdays which were the time wherein he was
working but upon talking with the mother, the husband is supportive of the child and bonded
with both of his child whenever he had the time.
Hindrances~

In the assessment there were no major hindrances that were observed. Upon
evaluating, the mother was very supportive and had no negative claim upon assessment. The
child on the other hand, was at first very shy since I was still someone knew to her so I had to
introduce myself and reassure her in order to gain her trust and make the assessment easier
and help generate more honest data. One of the hindrances also that I experienced was helping
the child to understand the assessment. Since the patient was still a preschooler, I used easy
words on her and mostly gesturing what we would be doing like for the test of ROM part.

Recommendation~

My recommendation for myself is that, I should prepare questions and sentences that
would make it easier for both the mother and child to understand the assessment. For the
recommendation for the mother, she should help her child to socialize by letting her play and
other means. Lastly, for the child, she should be exposed to other people in order to help her
build her confidence and social skills, she should also be playing with other children to not only
help her socialize but also for her to get the needed exercise she needs.
