Hypotension: a notable consequence of its mechanism of action. Patients typically experience worsened hypotension during the first dose, the effect tapering off thereafter. Persistant dry cough: attributable to the accumulation of kinins in lungs. Hyperkalemia: ACE inhibitors promote retention of potassium ions. Patients potassium levels should be monitered. Clinical considerations When we talk about the clinical pharmacology of ACE inhibitors, we need to think about the following factors 1. The ACE inhibitors should be used with caution in patients with renal impairment. 2. For chronic kidney disease lower doses should be used and their effects monitered. 3. ACE inhibitors are classified as pregnancy catergory D, meaning there is a positive evidence of risk. 4. Potassium supplements with ACE inhibitors are used with cautions. 5.
