JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

We, NIDA EBOL and VERNAN ALABATA, both of legal ages, Filipinos,
and residents of Camino Nuevo, Zamboanga City, Philippines, after having been
duly sworn to an oath in accordance with law do hereby depose and state:

1. That we know the person of MARY CLAIRE INFANTE BONIFACIO

who was born on October 14, 2020 to her parents Annaliza Infante and
Glady Bonifacio;

2. That due to inadvertence on the part of her mother as well as the

prevailing conditions at that time, the birth of MARY CLAIRE INFANTE
BONIFACIO was not registered with the Civil Registrar;

3. That we are aware of the foregoing facts as we are neighbors and close
acquaintances of Glady Bonifacio and Annaliza Infante

4. That we are executing this affidavit to attest to the veracity and

truthfulness of the above facts, in furtherance of the registration of the
Certificate of Live Birth of MARY CLAIRE INFANTE BONIFACIO
