AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MARIA M. RIAMA and PEDRO E. ROPED, both of legal age, Filipinos, and
residents of Zone 1, San Fabian Street, Brgy. Del Monte, Quezon City and No. Zone 2,
San Fabian Street, Brgy. Del Monte Quezon City, after having been sworn to in
accordance with law hereby depose and say:

1. That we personally know JUNMAR M. DELA CRUZ who is the son of spouses
Juan Dela Cruz and Mara Makiling Dela Cruz;

2. That we know to the best of our knowledge that JUNMAR M. DELA CRUZ
was born on 12th day of May 2015 in Ospital ng Quezon, Quezon City, Metro
Manila;

3. That the birth of JUNMAR M. DELA CRUZ was not reported and registered in
the Office of the Civil Register of Quezon City due to inadvertence and
negligence of his parents;

4. That we personally know the foregoing facts because we are long time
friends and neighbors of Juan Dela Cruz and Mara Makiling Dela Cruz and
their families;

5. That we are not related to JUNMAR M. DELA CRUZ either by affinity or

consanguinity and has no interest in whatever this affidavit could be of use
either directly or indirectly;

6. That we execute this Joint Affidavit to attest to the truthfulness of the

foregoing statement and to support the application for delayed registration
of the birth of JUNMAR M. DELA CRUZ with the office of the Civil Register of
Quezon City, and for whatever legal purpose this affidavit may serve.
