 At the end of this lesion, all student should be able to:


1. Define the planning in nursing process
2. Explain the purposes of planning in nursing process.
3. State the components of the planning in nursing process.
4. Describe the process of developing planning in nursing process.
5. Demonstrate the implementation of planning in nursing care
plan.

 “ Identification of nursing actions for achieving goals or desired outcomes of


nursing care.”
Planning
Third step of the Nursing Process
 This is when the nurse organizes a nursing care plan based on
the nursing diagnoses.
 Once nsg diagnosis are identified, prioritizing of selected nsg
diagnosis need to be occur to determine care priorities.
 Nurse formulate goals to help the patient with their problems
 Expected outcomes are identified
 Interventions (nursing orders) are selected to aid the patient
reach these goals.

 During this phase, the patient’s goals, strategies and outcomes


are determined.
 Outcomes may exist for either the nurse or the patient it is
important to remember that the desired outcome is what the
patient will achieve, not the nurse.
 Writing an outcome statement is to describe the objective in a
comfortable , understandable way. Identify either the outcome is
clearly defined.
 Planning is deliberative, systemic phase of the nursing process that
involve decision making and problem solving
 The end product of the planning phase is a patient care plan.

Outcomes / goal setting involved different time spans .


long term – indicates overall direction of behavior.

Intermediate / short term - shorter range so as to


more specifically affect the nature of the interaction and
activities.

TYPES OF GOAL TIME

 Short term
 An objective behavior or response, nurses expect the
patient to achieve in a short period of time usually in an
hour or less than one week.
 Example:
Goal: Patient body temperature will be reduce
to normal range (36.4°C -37.2°C ) within
2 hour nursing intervention given.
TYPES OF GOAL TIME

 Long term
 Indicates overall direction of an objective behavior or
response, nurses expect the patient to achieve in a longer
period of time possibly over several days, weeks or
months.
 Example:
Goal: Patient will not develop pressure injury
during hospitalization.

This method assist the nurse in identifying patient
outcome more effectively.

Example : Outcomes /goal =


SMART
Patient body temperature will be reduce to
normal range ( 36.4°C -37.2°C ) within 2 hour
nursing intervention given.

Patient will have normal elimination with good


soft stool after 24 hours nursing intervention
given.
EXAMPLE
Nsg diagnosis : Acute abdomen pain related to irritated mucosa from acid
secretions

Subjective data
Patient complained of pain and asked for pain medicine before morning assessment.

Objective data
Pt placing hand on abdomen, guarding.
Patient pain score 8/10

Short Term:
Patient will rate pain as a 2/10 or less at the end of the shift. OR
Pain will be reduce in 2 hrs nursing intervention given.

Long Term:
Patient will rate pain as a 2/10 or less at the time of discharge and will be sent home
on low dose of oral pain medications.
Planning-select interventions

Interventions are selected and written.


The nurse uses clinical judgment and professional
knowledge to select appropriate interventions that will aid
the patient in reaching their goal.
Interventions should be examined for feasibility and
acceptability to the client
Interventions should be written clearly and specifically.
