THAT THE PARTY OF THE SECOND PART visit hours service rendered with the Tollowing deliverable: provide check up to students, teachers staffs and other persons ara cot reports from the visit/check up conducted and will sign on the required documents of the corporation for the reports that needs a physician THAT THE PARTY OF THE SECOND PART guarantees that she possesses the required Competence, aptitude, habits and skills as well as the requisite educational background and training; ‘THAT THE PARTY OF THE SECOND PART shall not assign, directly or indirectly, this contract to any third party; ‘THAT the parties agree on other services and fees not specified herein shall be subject to mutual agreement; ‘THAT THE PARTY OF THE SECOND PART shall not entitled to Social Services (SSS, PAGIBIG,PHILHEALTH) benefits; " ‘THAT the appropriate withholding tax for the PARTY OF THE SECOND PART shall be deducted from the fee.
