Expected learning outcomes:
1. Understand and discuss patient-centered care in older patients (Refer Smith et al.,
Pharmacy 2019, 7, page 101)
2. Describe medication related problems, provide suggestions and communicate with
other relevant health care professionals regarding the clinical case.
3. Apply knowledge and skills of various types of drug interactions to resolve patient
problems
4. Provide advice and counseling for a patient in an out-patient setting.
5. Consider specialist referral when needed.

Case scenario:
Mrs. Bhaigi is a 71- year-old female weighing 65kg and is 1.5m tall. Her General
Practitioner is concerned about her compliance and has asked to undertake a Home
Medicines Review.

Medical history:
Hypertension (since 10 years ago) is well controlled. Last BP reading 130/70mmHg
Suffers from reflux oesophagitis
Occasionally requires pain relief for knee pain.
Has no other risk factors for cardiovascular disease.

Current Medication:
Her current medications are:
Atenolol 50mg mane
Enalapril 10mg mane
Ranitidine 150mg bd
Piroxicam 20mg prn for knee pain

On questioning and discussing her medications you learn that she is also taking a
multivitamin (1 x Daily Vits tablet) and sometimes uses ibuprofen tablets (200mg)
which she buys from another Pharmacy down the road. In addition she also admits to
sometimes forgetting to take her antihypertensive medications.
