Title of Study: PARENTS INVOLVEMENT AND STUDENT ACADEMIC PERFORMANCE

Project Description: You are invited to participate in a study that researches study the
relationship between parent involvement to the academic performance of their child.
Your participation in the study can help the parent to understand their involvement in the
education of their child. Survey questionnaire is the tool that the researcher used to
identify the level of involvement of the parent and how it affect to the academic
performance of their children.

Your participation in the study will entail the following:

A survey questionnaire
The accomplished questionnaires which will be retrieved by the researcher through retrieval of the

module will be summarized, tabulated and sensitized by the researcher with the use of befitting statistical

procedure. Aside from the responses of the parents, students, and teachers, general average of the students

in their report card will be done to support the data gathered. The researcher will provide questionnaire

through the distribution of module to reach the participants. The answer of the participants to the

questionnaire which they were made to accomplish plus the general average of the students in their report

card will compose the primary source of data needed in this study.

Risks and Benefits

There are no risks involved in participating in this study other than those encountered in day – to
– day life or the possible risks or discomfort of the study are minimal . You may feel a little
uncomfortable / sensitive on survey question.

Data Storage
You will not be identified by name at any point in the research findings. The data will be securely
stored on an encrypted hard drive and coded. The only individual with access to the data will be
the researcher.

How the research will be used

The research will be used for a thesis submitted in partial fulfillment of the requirements for the
degree of Master of Arts in Education Major in Administration and Supervision. The
research will also be used by the teachers, students, and parents’ performance in North B
District.

I would like to ask your permission to allow me to conduct a survey among the teachers,
students and parents in your division. This is in view of my thesis, entitled, “Parent Involvement
and Students Academic Performance”. Attached herewith is the survey questionnaire for this
study.

The survey would last only about 10-15 minutes and would be arranged at a time convenient to
the teachers’ schedule (e.g. during break). Participation in the survey is entirely voluntary and
there are no known or anticipated risks to participation in this study. All information provided
will be kept in utmost confidentiality and would be used only for academic purposes. The
names of the respondents and the name of school will not appear in any thesis or publications
resulting from this study unless agreed to.

After the data have been analyzed, you will receive a copy of the executive summary. If you
would be interested in greater detail, an electronic copy (e.g. PDF) of the entire thesis can be
made available to you.

If you agree, kindly sign below acknowledging your consent and permission for me to conduct
this study/survey at your division and return the signed form on an enclosed envelope.
