AFFIDAVIT OF NON-FILING OF INCOME TAX RETURN

I, RESA M. SAB-A, of lega age, Filipino and resident of 67 Halcon II St., Bgy. Sta.
Teresita, Quezon City after having been duly sworn to in accordance with law, depose and say:

1. That I am the mother of OWEN MARK S. CLAMAYAN a student of Maranatha

Christian Academy Manila Campus and applying for a scholarship grant;

2. That I am at present not gainfully employed and just depend on a small sari-sari store
which has no fixed income derived from the same;

3. That one of the requirements therein is the Income Tax Return which I have noe;

4. That it is my wish that the Bureau of Internal Revenue will issued a Tax Return
Exemption for use in the said application for scholarship;

5. That I am executing this affidavit to attest to the veracity of the foregoing for all intents
this may be of legal worth.
