Assessment 1 Create your Personal Stress Management Plan

Assessment Description:
Create a personal stress management plan to use during this period of study and when you are on
practical placement.

Assessment Method / Conditions (if applicable):


Together with Assessment 3 (Reflection on stressful situations during practical placement), this will
create a portfolio of evidence.

Assessment Venue:
You will commence the assessment in the classroom and complete it in your own time.

Assessment Time/duration:
Commence during class on 26th July 2019.

Due Date:
2nd August 2019- by 5PM- submit via Blackboard

Assessment Instructions:
Your stress management plan must be designed around your own responses to stress and your own
life situation.
Use the attached template and use 12pt Arial font.

• spend 30 hours per week having fun and/or relaxing
• write a daily evaluation so as to analyse my long term life style trends
