INTRODUCTION
Title : Fatal incident in TT parking area
Location : TT Parking Area
Loss/ Outcome : One fatality
INCIDENT
At around 16:00 hrs, driver of a TT, reported unwell by witness, lied down in front of the front wheel
of another TT parked in TT parking area to take rest and went to sleep. The driver of second TT did
not notice the victim sleeping in front his TT. At 16:53 hrs, the accused driver started and moved the
TT without acknowledging the TT driver (victim) sleeping in front of the TT. The victim’s head was
crushed under the front wheel. Immediately, location officials were called by security staff but the
victim was already dead.
OBSERVATIONS
 Victim slept in front of the TT although driver rest room is available.
 Security guard noticed the driver sleeping in front of the TT but neither he nor security supervisor
considered this to be an unsafe act which needed to be immediately stopped and reported.
 Security guard was using 2 wheelers in the TT parking area.
 TT driver started the TT without checking the surrounding area of the TT.
 TT was not parked in the slots provided for TT parking.
 There was no helper in TT.
 Victim’s TT was driven by helper who was without Photo pass and valid driving licence including
training for transportation of Hazardous good from RTO authorised agencies and allowed in the
terminal by security staff without appropriate approval.
ROOT CAUSE
 Non adherence to SOP (violation of many of the requirements of do’s & don’ts of TT parking area
and TT checking at gate).
 Casual attitude, non-monitoring of CCTV for unsafe acts/ conditions.

 Ineffectiveness of training to security guards and TT crew.
RECOMMENDATIONS
 Training to be imparted to all the drivers regarding the initial checks before starting TT/ safe
driving/ defensive driving and on unsafe acts/ conditions of TT parking area.
 Training to security guards on unsafe acts/ conditions of TT parking area.
 Supervision of parking area to be strengthened. Security supervisor/ guards to be trained on use
of CCTV for monitoring of unsafe acts/ conditions.
 Security guards to be sensitized to report/ take action on unsafe acts/ conditions.
 SOPs (for TT checking at gate) and do’s & don’ts of TT parking area to be displayed and strictly
adhered to.
 Only drivers with Photo passes and valid driving licence for transportation of Hazardous goods
endorsed by RTO and C&A verification to be allowed to drive the TT inside the Terminal.
 Unauthorised access (of pedestrians & 2 wheelers) to TT parking area to be stopped.
