I, J LUIS TRONQUED BURGOS, of legal age, Filipino, with business

address at No. 39 Visayas Ave., Vasra 1, Quezon City, after having been duly
sworn in accordance with law, depose and state that:

That I am the registered owner/proprietor of a business named PAKBET

DIGITAL VIDEO SERVICES a business duly organized and existing under the
laws of the Republic of the Philippines with business address at No. 39 Visayas
Ave., Vasra 1, Quezon City;

That my said business name did not operate since March 30, 2020 up to present
due to pandemic;

That I hereby under oath that my said business has no income and no operation
since March 30, 2020; 

That I am executing this affidavit to attest to the truth of the foregoing facts and

for whatever legal intents and purposes this may serve.
