STREET PAEL SUBDIVISION BRGY. CULIAT QUEZON CITY 1128, after having been duly sworn in
accordance with the law, do hereby depose and say:

1. That I am the owner of TECHNATION TELECOMMUNICATION SERVICES;

2. That as such owner, I hereby certify and attest to the fact that TECHNATION
TELECOMMUNICATION SERVICES, a duly accredited contractor of PLDT Co., has paid all the
following Commission owing to its Sales Agent:

SME 120% Commission for the period of December 1-31,2022

That such payments are in all respect and in accordance with requirements of the Labor Code and
applicable rules and regulations.

3. That I have executed this affidavit in order to attest to the truth of all foregoing and for all legal
agents and purposes this may serve.
