CONTRACT OF SERVICES BARANGAY

(BRGY. CONTRACT TRACER)

This CONTRACT made and entered into the 1st day of SEPTEMBER, 2022 in Barangay Pasong
Tamo, District 6, Quezon City, Metro Manila, Philippines by and between:

BARANGAY PASONG TAMO, DISTRICT 6, QUEZON CITY, METRO MANILA

A government entity organized and existing under and by virtue of laws of the Republic of the
Philippines with office address at Philand Drive, Tandang Sora Avenue, Quezon City, Metro
Manila and herein HONORABLE MAE A. TAGLE in his capacity as Punong Barangay herein
reffered to as PARTY OF THE FIRST PART.

All of legal age, resident of Barangay Pasong Tamo, District 6, Quezon City, Metro Manila and
herein referred to as the PARTY OF THE SECOND PART.

~WITNESSETH~
THAT, for and in consideration of the sum of SIX THOUSAND FIVE HUNDRED PESOS
(PHP6,500.00) each or equivalent to NINETEEN THOUSAND FIVE HUNDRED PESOS ONLY (PHP
19,500.00) per month and herein agreements, the PARTY OF FIRST PART, HEREBY agrees to
hire the services of the PARTY OF THE SECOND PART as a BARANGAY CONTACT TRACER;

WHEREAS, the Barangay Council of Pasong Tamo, Quezon City has appropriate the sum of ONE
HUNDRED SEVENTEEN THOUSAND PESOS ONLY (PHP117,00.00) in the six monthly Barangay
Annual Budget for Fiscal Year 2023 as embodied in the specific guidelines allocated for
maintenance and Other Operating Expenses under Special Projects for Peace and Order for
contract services rendered to Barangay Pasong Tamo for the period from JANUARY 02, 2023 to
JUNE 30, 2023 for the above stated projects;

WHEREAS, THE PARTY OF THE FIRST PART may terminate the services of any of the PARTY OF
THE SECOND PART for loss of trust and confidence and similar grounds at any time and said
PARTY OF THE SECOND PART shall not be entitled to the compensation due for the remaining
period;

WHEREAS, the effectivity of the CONTRACT will commence on JANUARY 02, 2023 to JUNE 30,
2023;

NOW THEREFORE, in view of the foregoing, for and in consideration of the herein mutual
covenants and undertakings herein provided, the parties hereto have agreed on the terms and
conditions stipulated as follows:

1. THAT the PARTY OF THE SECOND PART, namely:

Will receive SIX THOUSAND FIVE HUNDRED PESOS (PHP6,500.00) a month or a total of ONE
HUNDRED SEVENTEEN THOUSAND PESOS ONLY (PHP117,000.00) payroll from JANUARY 02,
2023 to JUNE 30, 2023.

2. THAT the PARTY OF THE SECOND PART, shall strictly follow the terms and conditions of
this agreement.

3. THAT, the PARTY OF THE FIRST PART, shall determine the personnel of the PARTY OF
THE SECOND PART and have the right to suspend, replace and appoint if ever necessary
so as not to hamper the operation of BARANGAY CONTACT TRACER, Special Projects
and Activities of the Barangay and the Barangay Council may also recommend to the
PARTY OF THE FIRST PART for such acts which will be prejudicial to the Barangay;

4. THAT, the PARTY OF THE FIRST PART shall not and will not be liable for any violation of
labor laws and other liabilities incurred on or during the performance of the duties and
responsibilities of the PARTY OF THE SECOND PART; and

5. THAT the PARTY OF THE FIRST PART or his representative shall cause the preparation of
the Daily Wage Payroll and the necessary disbursement vouchers to effect collection of
their monthly salaries upon the approval of authorities concerned.

and voluntary act and deed, I hereby certify that both parties are personally known
