AFFIDAVIT OF CANCELLATION

I, JOHN DARYL A. DELA CRUZ, of legal age, single, Filipino citizen, and with
business address at #46 BMA St., Brgy. Tatalon, Quezon City, after having been duly
sworn to in accordance with the law hereby depose and state;

1. THAT, I am the registered owner of J.A.D.E. 46 AUTO PARTS AND

ACCESSORIES SHOP with DTI Certificate Number 4739179 and was
registered last MARCH 01, 2023;

2. THAT, I am no longer interested in continuing my business that in connection I

have no financial obligation at this time of cancellation;

3. THAT, all dues and obligations, whether from private and governmental, had
been paid and settled accordingly;

4. THAT, I executed this affidavit to attest to the truth of the foregoing

statements and that the same are true and correct to the best of my
knowledge and for the purpose of cancelling the business name permanently
closed by/due to city ordinance.
