AFFIDAVIT OF FAMILY CONSTITUTION

I, FELISA DELA CUESTA y NULUD, of legal age, widow, Filipino and a

resident of Caloocan, Balayan, Batangas, hereby depose and states under oath that:

1. I am the legitimate wife of the late DELFIN DELA CUESTA, the two of us
having been legally married on 14 June 1954 at Balayan, Batangas.

2. That we were blessed with six (6) children, including HELEN N. DELA
CUESTA-DELOS REYES, born on 26 January 1969, married to FELIX
DELOS REYES, who is currently residing at Via Calimno 54, Rome, Italy and
is now sponsoring/processing a Petition in my favor.

3. That the following are my other children. Specified also are the civil status, full
addresses and activities/occupation of each of them:

MARIETA DELA CUESTA-ATOG, born on 14 May 1959,

married to JOSE ATOG, presently residing at Central,
Caloocan, Balayan, Batangas. She is a full time housewife.

TERESITA DELA CUESTA-CABATO, born on 22 September

1958, married to ALAN CABATO, presently residing at Union
St., Balayan, Batangas. She is a full time housewife.

LOURDES DELA CUESTA-ALBAY, born on 3 November 1963,

married to MAURO ALDAY, presently residing at Jamaica
Subdivision, Caloocan, Balayan, Batangas. She is a full time
housewife.

FIDEL N. DELA CUESTA, born on 21 April 1967, married to

MARY ANNE DELA CUESTA, presently residing at Maria
Christina Heights Subdivision, Caloocan, Balayan, Batangas.
He is a part-time mechanic.

DANIEL N. DELA CUESTA, born on 3 January 1972, married

to WENIE DELA CUESTA, presently residing at Caloocan,
Balayan, Batangas. He works as a garbage collector of
Barangay Caloocan.
 4. I am executing this affidavit to attest to the truth and veracity of the foregoing
facts and in support of the application for petition visa that I have submitted
with the Italian Embassy.
