AFFIDAVIT

I, ELIZABETH BAZAR y TEGIO, Filipino, 52 years of age, widow

and presently residing at Manila, Rapu Rapu, Albay, under oath state:

1.

That I was married to the late ELMER BAZAR y VIAS and

our marriage had given us four (4) children, the youngest of whom
is ARLENE BAZAR y TEGIO;

2.

That our daughter ARLENE BAZAR y TEGIO was born with

the assistance of a local hilot on June 30, 1988 at a boarding

house in 1623 AB Yakal St., Tondo, Manila, where my late
husband and I were staying at the time;

3.

That years unfortunately passed and still we were not able to

register the birth of our daughter ARLENE BAZAR y TEGIO in

Manila;

4.

That I execute this Affidavit to attest to the fact of birth of my

daughter ARLENE BAZAR y TEGIO on at our boarding house in

1623 AB Yakal St., Tondo, Manila and for whatever legal purpose it
may be needed;

IN WITNESS WHEREOF, I hereunto set my hand this ___ day of
