AFFIDAVIT

I, MAJ CHRISTOPHER C MUGOT O-13492 (INF) PA, of legal age,

Filipino citizen, married and presently assigned with OG8, 4ID, PA stationed
at Camp Edilberto Evangelista, Patag, Cagayan de Oro City, after having
been duly sworn to in accordance with law, do hereby depose and say:

That, I was in possession of a Firearms License bearing Number

869522I1222074 duly issued by the authorities of the Philippine National
Police (PNP) and the same has already expired on December 9, 2009;

That, I intend to renew said Firearms License after its expiration

purposely for the continuous possession of it; However, due to the nature of
my work being a member of the AFP wherein I was reassigned from one
place to another, I was not able to renew it;

That, the above-mentioned firearm has been stored in a safety part of

my residential house in Brgy Iponan, Cagayan de Oro City, up to the filing for
the renewal of said license;

That, I am executing this affidavit to attest to the truth of the

foregoing, and to further inform the authorities concerned that said firearm
has not been involved in any crime nor used as an evidence in a pending
case before any court or proceedings and for whatever legal purpose this
may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 20 th
