The nursing goal for patients with stroke can be towards maintaining effective tissue perfusion,
preventing further complications, and enhancing coping. This is a nursing care plan for patients
with hemorrhagic stroke.

3 Activity Intolerance
Motor deficits are the most obvious effect of stroke. Symptoms are caused by the destruction of
motor neurons in the pyramidal pathways (nerve fibers in the brain and passing through the
spinal cord to motor tract.)

4 Self-Care Deficit
Motor deficit are the most obvious effect of stroke. Symptoms are caused by destruction of

spinal cord to the motor tract.) When this happens, activities of daily living can be impaired and
even self-care.

a. Definition. Cerebral vascular accident (CVA) (stroke) is the disruption of the blood supply to the brain,
resulting in neurological dysfunction.
(1) Hypertension.
(2) Previous transient ischemic attacks.
(3) Cardiac disease (atherosclerosis, arrhythmias, valvular heart disease).
(4) Advanced age.
(5) Diabetes.
(9) Increased intracranial pressure is a frequent complication resulting from hemorrhage or ischemia and
subsequent cerebral edema.

(9) Include patient's family and significant others in plan of care to the maximum extent possible.
