Restore Equity Status with 3 documents:

1. Ecclesiastical Deed Poll

2. Declaration
3. Certificate of Live Birth

Notarize 2 & 3.

Deed poll requires 2 neutral witnesses

 Deed to my personal estate

 Do this properly and I can claim my personal estate as a beneficiary
 This creates an ecclesiastical instrument
 Able to claim estate as beneficiary if do it properly
 Claims your BC registry #

Make them self-authenticating / official documents.

1. Notary + 2 witnesses (create self-auth. Document)

2. File document with county recorder

- Until they become self authenticating, courts won’t look at these.

- File these with 3 documents with county recorder as state property using declaration and COLB
- Write this with the submission:
o Identify it as an ALL CAPS name
o Request to change the spelling from ALL CAPS to family / given name
o Include that my birth was a result of my parent’s marital union – John Quincy (don’t
need last name because it’s included with mother / father)
 Include parentage mother and father, where they were born and when.
 Given name
 Place of birth
 Date of birth
o I’m no longer lost at sea, in the sea of commerce.
o I found my way back home
o I declare myself as a living woman using my proper name and ready to assert my
responsibility as a productive citizen.
o I no longer want to be referred to as ALL CAPS.
- 2 people must witness my deed poll and they cannot be related to me by blood or marriage
(1779 docs have this AVR)
o I include their addresses after mine on the deed polle
o All 3 of us sign in front of the notary and she puts her name and seal on it
o This document is now self-authenticating with those 2 signatures.

To go a step further…

- Record doc with county recorder and get (5?) copies back
- Send to the SOS
- Request a certificate of acknowledgement from the SOS
 o Will get an official seal on the documents on the seal
o Request 3 copies with SOS acknowledgement.
- Request 3 copies of SOS acknowledgement

From these 3 copies, create a petition for probate court

- This petition title caption: “administrative errors on the record for equity status” titled (a
petition for equity status)
- Include all 3 notarized exhibits (deed poll, declaration, COLB) including SOS seal (a separate
piece of paper)
- Include an order and decree for the judge to sign and that’s to request an official name change.
o Same information as stated in deed poll.
o Deed poll and BC and their SOS ____
- Appear on date given
o He may ask several questions.
o My goal is to correct the record
o There are administrative errors that I’m identified incorrectly.

Should probably now file with the Secretary of State make 3 copies then 5. File with the county
recorder as Estate Property.

Apply for a name change under ecclesiastical juris.

 Change from the ALL CAPS name to the proper spelling of your given name / family name.
