Informed Consent Form for KEY FACTORS AFFECTING THE EXCESSIVE DIGITAL EXPOSURE
AMONG MILLENNIALS IN DAVAO CITY: A COMPARATIVE STUDY

INTRODUCTION

You are invited to participate in a research study conducted by AVRIL MELYN LAO, ALTHEA
MHAXIN DE CASTRO, RIA NICA BAGUIO, CARL MARIE GOCON, ROSEANNE ENTORUM,
FLORENCE APARRI & THOMAS JAMES GATES, at the Joji Ilagan International Schools, because
you fit the inclusion criteria for informants of our study.

Your participation is completely voluntary. Please read the information below, and ask questions
about anything you do not understand, before deciding whether to participate. Please take as much
time as you need to read the consent form. You may also decide to discuss participation with your
family or friends.

If you decide to participate, you will be asked to sign this form. You will be given a copy of this form.

PURPOSE OF THE STUDY


The primary objective of this study is to gather data in assessing the key factors affecting the
excessive digital exposure among millennials in Davao City.

STUDY PROCEDURES
The researchers will adhere to a standard procedure and protocol for data collection, beginning with
getting permission from the Dean of the institution to conduct the research after being endorsed by
the adviser. Data gathering will be done through an online survey. The researchers will ensure that
you have received, reviewed, and signed the informed consent allowing the researchers to conduct
the survey and gather data. Once approved by the college dean, the online survey will then be
commenced. A list of predetermined, open-ended questions, which serves as a guide for the
researchers, will be utilized. During the survey, additional questions may not be asked.

POTENTIAL RISKS AND DISCOMFORTS


Due to the Covid-19 pandemic, the collection of data will be done through online survey and utilizing
Google Forms as to avoid face-to-face data gathering. This is to adhere to the Inter-Agency Task
Force (IATF) Guidelines and comply with the health and safety protocols to minimize risks for the
participants and researchers. The researchers will value and prioritize your health and well-being
during the course of the study.

POTENTIAL BENEFITS TO PARTICIPANTS AND/OR TO SOCIETY


This study will be beneficial to the participants in a way that you will be able to consider your feelings
and experiences, without judgment, and without those being used against you. The findings of this
study will also provide insights for the government by giving information on the digital exposure of the
millennials. The government will be aware about the millennials’ excessive exposure, enabling them
to recognize the key factors affecting the millennials’ digital exposure. Furthermore, by assessing
excessive digital exposure, this study will benefit the society by providing the assets and liabilities of
digital exposure among millennials.

DATA PRIVACY AND CONFIDENTIALITY


This study will strictly adhere to the Data Privacy Act of 2012. The principles of transparency,
legitimacy, purpose, and proportionality in collecting, retaining, and processing personal information
will be highlighted. In accordance with this law, the researchers will recognize and respect your right
to privacy. With this, your identity (e.g., individual name and academic information) will remain
confidential. Your anonymity and confidentiality will be preserved by not revealing your names and
identity in the data collection, analysis, and reporting of the study findings such that the researchers
will use codes and pseudonyms which do not represent your actual name. Moreover, the researchers
assure you that there will be no intrusion of privacy regarding the information provided and ensure
that the data collected from you will be safely stored and protected away from unauthorized
individuals.

VOLUNTARINESS OF PARTICIPATION AND RIGHTS TO WITHDRAW FROM THE RESEARCH


Your participation is voluntary. Your refusal to participate will involve no penalty nor loss of benefits to
which you are otherwise entitled. You may withdraw your consent at any time and discontinue
participation without penalty. You are not waiving any legal claims, rights, or remedies because of
your participation in this research study.

REIMBURSEMENT AND COMPENSATION


You will be given a heartfelt thank you as a sign of gratitude for the time and effort you will be sparing
in participating in the research and in helping the researchers accomplish the study.
