 Office: Registrar of Birth and Death

 Documents asked: Adhar, Matriculation certificate, Passport and a photograph.
 Time of processing: 5 days approx.
 Fee: Don’t remember exactly but it was a very nominal amount

I had also got this translated completely to English (just to be on a safer side though it wasn’t really
needed since all the important content on the certificate is in English). Along with translation, you need
to attach an affidavit from translator)

This is how my birth certificate looks like:

 Documents submitted for employment proof:

1. Employment reference from company’s head of the India office

2. Employment Agreement (the one that I signed while joining the company)
3. Form 26 AS for all the years that I worked with the Company. It mentions company’s name for
showing TDS on salary. It is available through income tax portal.
4. Salary slip for the first month and the last month. One salary slip for each year with company
was also furnished.
5. A couple of certificates that were provided as felicitation for my work were also provided.

account is solely held in my wife’s name and it is acceptable.

3. My mutual funds statements. I asked my AMC to furnish me my mutual fund statement for the
past 3 years which shows all the amount that I have invested with them. Its easy, just call them
up on the customer care and tell them your portfolio number, and they shall send it to your
email.
4. Spouse’s mutual fund statements.
