ISOLATION – MONITORING AND HANDOVERS

System
This SOP outlines the strategy of Monitor and Handover of the patients admitted in the Isolation Unit in
Cadus Field Hospital in Hol IDPs/Refugees camp.

Staff
The Team Leader, the Doctor on duty, and the Nurse designated for the patients in Isolation Unit are in
charge of implementing the measures described below.

Strategies

1. General:

• Establish a Nurse dedicated exclusively to the patients in Isolation for every shift of 24 hours.
This nurse will exclusively work with the patients admitted in the Isolation Unit and Ward.

• At every contact with the patients admitted in the Isolation unit or in the COVID ward,
the use of the relevant PPE is mandatory.
PPE: personal protection equipment (i.e. mask, eye protection, gloves, gown).

• The name of the Doctor on duty and the Nurse on duty in the Critical Care Unit will be written
on the monitor sheet “Monitor Isolation” at every shift of 48 hours.
Note. The document must be left outside the room and filled after the visit.

• To identify the patients who must be isolated and choose the appropriate type of isolation, see
“Protocol - Isolation and IPC Exceptional Measures”

• The patient in Isolation must not leave the Isolation Unit, nor receive any visit.
The only exception is small kids in need of maternal case: in this case the same rule must be
applied to the mother.

2. Monitor:

• Every 4 hours, report:


- Saturation (specify if on oxygen, and how many L/min)
- Heart Rate, Respiratory Rate, Blood Pressure, Temperature, Glasgow Coma Scale (GCS)
- Medications
- Nutrition (to survey that the patient is receiving food, and enough)

• Report when the samples (pharyngeal swap, sputum, blood culture, rachicentesis, or other
samples) are taken, or when an X-ray is performed.

3. Handover:

• Every 48 hours a copy of the report “Monitor Isolation” will be posted in the WA group
“Medical Handover”.
