STATEMENT OF DEFICIENCY
During routine site surveillance on Dated 24 Aug, 2021, It has been found that the construction team
installed the street lighting system 12M Lighting pole foundations at Modon Road without material
Receiving. Which is clear violation as per approved ITP No. SEPR-ITP-Q-03 Rev 0 Item No. 2.1. Construction
team is advised to provide the root cause and acceptable corrective action along preventive action within the
ACD.
ROOTCAUSE OF NON-CONFORMANCE
- Due to unavailability of compressive strength report of Lighting pole foundations.
-Construction team failed to provide the required documents for material Receiving.
