About the Document


The Health and Safety Management Plan (HSMP) is prepared in line with Sub Clause 1613
the Technical Specifications ERA for execution of the road construction works in a safe and
healthy manner during the construction phase of Bahir Dar-Tis Esat DB Road Project.

The Plan is a project specific document to be used by the Contractor and subcontractors
under the scope of the project. It will be subjected for update and amendment with
appropriate inputs from all responsible parties/ stakeholders depending on the status of
the construction site.

1 Introduction
1.1 Background and Purpose
The Government of the Federal Republic of Ethiopia (FDRE) continues its commitment to give the
Road Sectors the highest priority and is preparing more projects for funding by donors and from
the national budget. Individual road projects are selected for inclusion in the RSDP on the basis of
needs assessment and taking into account the level of traffic, road condition and access problem,
importance of roads, etc.

As part of its continuing effort to provide transportation service and to support economic development in
the country, the Ethiopian Roads Authority has awarded MELCON Construction Company for the
construction of Bahir Dar –Tis Esat design and Build road project as well as construction of spur
walkway to Tis Abay Falls. The construction of the road project includes Earth work, structures,
pavement, different ancillary works and construction of irrigation structures.

It is our plan that to complete the construction activities within the contract time and quality required by
the project. In light of this to complete the project within the allocated time and quality it is mandatory to
have a health and safety plan for safe execution of work during the construction phase of the road
project.

The purpose of this plan is to provide a framework describing how the contractor manage and
implement the health and safety aspects of the road project.

1.2 Objective
The objective of this HSMP is to define the Health and Safety scope of work, requirements, and program
that will be followed on the project.

The Plan has been prepared to:

 Provide instruction and guidance to the project team, including subcontractors, on the
development and implementation of project specific health and safety requirements;

 Identify and incorporate safety considerations as related to construction works;

 Inform Subcontractors in advance of the health and safety requirements that will be
strictly enforced on the project;

 Establish roles and responsibilities associated with Health and Safety;

1.3 Project Description
The project road is located in the Northern part of the country, and lies entirely in the Amhara
Regional State of the Federal Democratic Republic of Ethiopia, particularly, in Bahir Dar Zuria
Wereda of West Gojam Administration while the end point (Tis Abay town) is under the administration
of Bahir Dar special zone. The estimated length of the design and build road is 21.2km with an
additional 1.75km spur walkway to Tis Abay Falls.

2 Essential Legal Provisions
Among the Legal requirements of the country, the Occupational Safety and Health Directives of the
Ministry of Labor and Social Affair/MoLSA/ of 2007 is most relevant; and hence part of it
considered as vital in this case are presented in this section.

2.1 General Duties of Employer


 The employer shall, in so far as is reasonably practicable, make arrangements for the
regular inspection of all structures, excavations, machinery and equipment to ensure that
those are capable of withstanding the stresses likely to be imposed upon them and of safely
performing the functions for which they are used;

 The employer shall ensure that necessary protective devices and clothing are provided to
the workers and that those are used for the safety and health of his or her workers;

 The Employer should create a conducive working environment in general, an incentive


mechanism, for workers who have achieved the intended target(s), depending on the
magnitude and nature of the achievement(s);

 The Employer shall ensure that safety procedures are followed at all workplaces.

2.2 General Rights and Duties of Workers


 Every worker shall, as a right, be informed, instructed and trained by the employer in
safety measures and procedures followed at the workplace.
 Every worker shall:

 Make proper use of all necessary safeguards, safety devices and appliances designated
and provided for his or her protection by his or her employer, or required under to his
or her employer or supervisor a hazardous condition that may come to his or her
attention.

 Report immediately to his or her employer or supervisor a hazardous condition that


may come to his or her attention.

2.3 Type of Works Prohibited for Young Workers


 works that endanger the safety, health and life of the young workforce

 Limits to the weight of loads to be carried, loaded, unloaded, drugged, pulled, pushed, etc by
hand;

 Limits to usage of rails.

2.4 Works that Affect Women


Such works prohibited to be undertaken by women could broadly be classified as:

 Handling of heavy weight loads,

 Works related to earthmoving, underground works, blasting, cleaning underground structures, etc;

 Hazardous works in metallurgy and handling of harmful


substances; Dangerous electroplating works;

 Limiting weights lifted by hand, wheel barrow, two - wheeled carriages, three - or more -
wheeled carriages and usage of trails. There are also listed types of works not meant to be
undertaken by pregnant women.

2.5 Causes of Accidents


The following are the major causes to which accidents are usually associated at road construction and
maintenance work places:

 Application of explosives

 Ladders

 Scaffolds

 Prefabricated and wooden scaffolds

 Bridges

 rock excavation

 Rock- crushing plants (Crushers)

 Removal of excavated material

 Falling ground

 Fire

 Traffic accidents

2.6 Hygiene and Sanitation
 Regular awareness sessions for workers on basic sanitation and its use;

 Provision of adequate number of (and of reasonable sanitary standard) toilets;

 Provision of protective materials such as misquote nets as required;

 Safe disposal of liquid and solid waste

 Kitchens should be clean

 Dust should be suppressed to the extent possible

3 Roles and Responsibilities for HSMP


The health and safety plan of the project muse be held by experienced experts who will have the
role and authority to control HS issues as well as to avoid the possible occurrence of any
undesirable event. The possible roles and responsibilities of key staff with regards to HS
management are described below.

3.1 Project Manager


The PM is responsible for ensuring that the overall project is executed fully in accordance with
applicable HS requirements of the road project. Specific requirements include:

 Ensure that adequate resources are assigned to meet the objectives health and safety plan;

 Ensure that the project HS Plans and Procedures are established and implemented;

 The project manager will chair all meetings related to health and safety and ensure all
matters brought out in the meeting are solved.

 The Project Manager has the ultimate responsibility for reviewing suitability of and approval of
the project HS plans and also monitoring its implementation and effectiveness.

3.2 Health and Safety Officer


The main responsibility of the health and safety officer is to ensuring that HS requirements are met
during the Construction phase of the project. This includes;
 Coordination with the employer’s representative and the various stakeholders such as Bahir
Dar town and Bahir Dar Zuria wereda health offices;

 Managing the overall Health and Safety aspects of the project based on recommendation and

in receipt of instruction from the Engineer;

 Organize Training programs,

 Monitoring the safe execution of work and consistent use of personal protective equipment
(PPEs);

 Proper use and storage of hazardous and toxic materials;

 Submission of reports to project manager on all matters pertaining to health and safety
status of the project.

3.3 Site Engineers


The site engineer/foreman will be responsible for the following:
 Implement and maintain HS controls on site;
 Report any activity, that has resulted or has the potential to result, in Health incident
immediately to safety officer;

 Record health and safety status and incidents and submit it to the safety officer on daily basis,

3.4 Sub-contractors
 All the sub-contractors staffs and workmen shall adhere to all rules and regulations
pertaining HSMP and follow instructions forwarded from the safety officer of the project;
 In case of any violation of rules and safety by any of his workman at the site, the sub-
contractors will be held responsible.

3.5 Construction Workers


 It shall be noted that every worker must comply with the requirement of HSP rules and regulations;

 For a safe working atmosphere, it is very important that the employees cooperate with the
company in implementing the safety requirements of the road project;

 The Employee should adhere to a safe working practice. Nobody should work at an unsafe
place. Everybody should wear the required personal protective equipment (PPE), follow safety
rules and practices and inform their supervisors about any unsafe condition.

 All employees are responsible in reporting hazards and accidents/ incidents.

4 Health and Safety Facilities
4.1 General
For the construction phase of the road project, MELCON has the responsibility to arrange occupational
health and safety programs and facilities for all workers and staffs entitled to be on the site, so as
to prevent any accidental risks and injuries due to the project work sites.

MELCON is compiling with all Ethiopian Laws with regard to provisions of health and safety facilities
and devices as per the contractual agreement of the project. Adequate training and awareness creation
will be given to the workers before engagement on the work sites. All drivers, machine operators,
blasters, traffic safety guides and personnel, electrical, mechanical, welders etc. will be oriented and
made aware of the occupational safety issues.

4.2 Occupational Safety


MELCON will provide the necessary protective clothing and safety devices to each worker depending
on type of work and on the characteristics of the site. The use of such protective facilities and
clothing is mandatory while workers are on duty; the proper implementation of this requirement will be
supervised and controlled by site engineer, and action will be taken accordingly. The following
safety plans will be implemented.

4.2.1 Personal Protective Equipment’s


MELCON will install fire extinguisher at facilities and plant sites; garages and workshops, camps,
vehicles, and major establishments. Personal protective equipment and clothing will be provided
to workers according to the specific work characteristic.

 High visibility vests for workers directing traffic;


 Protective boots, gloves, masks for the workforce at bituminous pavements;
 Protective foot wear, gloves and dust mask for workers undertaking screening, crushing or
grinding,
 Ear protection and dust mask for the workforce engaged at crusher plants, at rock drilling
or by using vibrating equipment such as rollers and compactors.

4.2.2 Offices and Accommodations
Good housekeeping practices in Offices and accommodations are an essential safety function
that contributes to efficient operations, better productivity and reduction in accidents, improving the
environment and enhancing workers moral. The following occupational safety programs will be
implemented at offices and accommodation;
 Janitors will be assigned at office and dormitory;
 Removal of garbage shall be carried out on daily basis;
 Adequate Waste collection bins will be provided at offices and dormitory,
 Unwanted materials shall be removed from workplace on daily basis,
 Electrical systems will be laid as per safe work methodology etc;
 Fire extinguishers will be installed and checked on monthly basis.

4.2.3 Health and First Aid Facilities


MELCON will establish first clinic at the main camp site (km 7+400 LHS) equipped with
professional nurses; and first aid kits at work sites. The Company will provide basic health service
and facilities as per the standards for such work site clinics (table 4-1). A stand by vehicle will be
assigned at construction site for emergency purpose.

4.2.4 Coronavirus (COVID-19) Preparedness Plan


Basic on the current status of coronavirus pandemic, MELCON will take all necessary
precaution measures and implement them as per the guidelines of the Ministry of Health.
Accordingly, the following measures are recommended:

 Provision of sanitary products such as Soap, Detergents, masks, alcohols and sanitizers
at contractor’s and consultant’s offices as well as at camp construction site.
 Checking all staffs for signs/symptoms of the virus using available test kits e.g Infrared
thermometer. This can be done in collaboration with Bahir Dar health centers;
 Avoid unnecessary meetings and overcrowding;
 If agreed with employer’s representative, the contractor’s RFI shall be sent online.
 Training for laborers shall be carried out in collaboration with Bahir Dar health centers.

5 Training and Education


5.1 General
A total safety culture cannot exist without a work environment that supports ownership. The
expectations of each employee and the role that the individual plays in achieving health and safety
excellence are to be communicated to all employees and are vital parts of the HSM plan.

In addition prior to an individual being allowed to work it is essential that they are trained and
educated on the project needs. During the duration of the contract this shall be both assessed and
audited on a regular basis and where found or identified corrective measures shall be put into place. It
is therefore proposed that a comprehensive induction and training program is established that prepares
the employees for working on the job. At each of the Project locations, the field representative will have
the responsibility of ensuring that the following are place as a minimum:

 Camp rules.
 Fire precautions and appropriate fire extinguishers.
 First aid provision and arrangements for competent persons to administer.
 Emergency and routine hospitalization facilities.
 Environmental incident response precautions and equipment.
 Project HS Policy, goals and targets.
 Incident reporting and investigation.
 Key personnel and their contact details.

5.2 Training
5.2.1 Orientation Training
All personnel of the main contractor and subcontractors are subjected to general information
program carried out by the Contractor’s HS unit. Workers who did not attend to these trainings cannot
work on site. All employees, regardless of the level in the organization, must receive health and
safety orientation training. Orientation on health and safety must be completed within the first two
weeks of hire. Components of training will include:

 Explanation of project and of employee’s duties.

 Requirements for use of personal protective equipment.

 Location of first aid, fire extinguishers, fire exits, telephone and emergency numbers.

 Accident reporting and emergency procedure details.

 Workplace Violence and Harassment training

5.2.2 Periodic Safety Trainings


Periodic safety orientation will be conducted every six months by the safety officer at the work
place according to the primary safety orientation program with all employees regardless of their
education, qualification and service time in the profession or job.

Training in the use and maintenance of PPE


All employees receive training on the use and maintenance of personal protective equipment
required when performing work on a site, garages, crusher, asphalt plant, concert mixing and precast
manufacturing Training is delivered prior to the start of any task or job function that requires the use of
personal protective equipment.

Required training will include but not be limited to:


 Recognition of the need of PPEs,
 Proper use of personal protective equipment,

 Selection of appropriate PPEs,

 Proper handling of PPEs,

 Workplace inspections

5.3 Health and Safety Meeting
HS communication and meetings are important for coordination, education and training. They are also
vital for stressing the importance of HS as an integral part of the work activities.

HS meetings will be held at all levels on a monthly basis with the participation of key personnel
related to each phase and location of the project. Project Management Teams shall conduct these
meetings at an agreed and fixed date and time on monthly basis.

Furthermore, HS meetings shall be scheduled, agreed and conducted between the Employer
Representative and Contractor.

6 Hazard Identification, Prevention and Risk Control


The primary means of protecting the safety and health of employees must be measures to eliminate
work place risks at source: firstly by technical means; secondly by organizational provision; and
thirdly by collective protection measures. Where these measures are not sufficient, Personal Protective
Equipment (additional protection to the individual) must be used to protect against hazards which are
unavoidable. In addition, the following approach to managing hazards and risks will be adopted.

6.1 Safe Work Practices


The Employer and Contractor Safe Work Practices, and applicable local law and regulation shall
be followed at all time for a healthy, safe and environmentally friendly execution of the job
activities. The specific requirements mandatory for safe work practices and activities related to
hazards include the following sub-sections

6.1.1 Operation of Equipment and Machinery


Contractor employees shall be trained in the operation, inspection, and maintenance of the equipment
and the safety features and procedures to be utilized during operation, inspection, and
maintenance of the equipment. This training shall be based on the equipment operating manual and
the hazard analysis for the activity.

Before any machinery or mechanized equipment is brought on site and placed in use for the first
time, it shall be inspected and tested by a competent person and certified to be in safe operating
condition and documented on a Contractor Equipment Inspection Form.

All machinery and equipment shall be inspected daily (when in use) to ensure safe operating conditions.

The Contractor shall designate competent persons to conduct the daily inspections and tests. Tests
shall be made at the beginning of each shift during which the equipment is to be used, to
determine that the brakes and operating systems are in proper working condition and that all
required safety devices are in place and functional. Inspections and tests shall be in accordance
with the manufacturer’s recommendations and shall be documented. Records of tests and inspections
shall be maintained at the site by the Contractor, shall be made available upon request, and shall
become part of the official project file. Inspections shall include as a minimum;

 Control and drive mechanism’


 Proper fluid levels
 Safety devices
 Hooks and safety clips for cranes, fork lifts, hoists and other lifting devices
 Tire condition
 Ground conditions, tonnage and outriggers (level) for cranes, fork lifts, hoists and other
lifting devices
 Load chart and hand signal chart
 Any dirt or contaminants
 Temperature and pressure of the equipment
Whenever any machinery or equipment is found to be unsafe, or whenever a deficiency that
affects the safe operation of equipment is observed, the equipment shall be immediately taken out of
service and its use prohibited until unsafe conditions have been corrected. A tag indicating that the
equipment shall not be operated, and that the tag shall not be removed, shall be placed in a
conspicuous location on the equipment.

Machinery and mechanized equipment shall be operated only by designated qualified personnel
wearing all applicable PPEs. It shall not be operated in a manner that will endanger persons or
property, nor shall the safe operating speeds or loads be exceeded. Persons who take prescription
medication should not operate such equipment if such medication may impair their judgment or
reflexes. Utilize equipment only for the purpose for which it was designed and in accordance with
the manufacturer’s instruction and recommendations.

Modifications, extensions, replacement parts, or repairs of equipment shall maintain at least the
same factor of safety as the original equipment.

6.1.2 Welding and Cutting


Compressed gas cylinders will be transported, stored and used securely fixed in the upright position.
When not in use, they will be stored on a cart in a specially designated area with valve caps on. Longer
storage of gas cylinders will be in covered sheds, covered roof and enclosed in three sides,
separated by a fire resistant wall. When lifted by crane, holders or trolleys will be used of standard
type. Ground return clamps will be attached to the work piece. Cable runs will be as short as
possible and joints, if any will be with approved connectors. The generator will be equipped with an
approved fire extinguisher. Electrodes will be removed from the holders when not in use. Holder will be
in good condition and cables will have no defects.

Welding presents possible molten slag fires. Fire extinguisher will be placed near the welders at all times.

A hazard may develop from flying particles during welding. Proper shielding and masks will be provided
for the welders and the fabricators.

Cylinders will be capped at all times when not in use. At the close of the business day, cylinders
in use will have the gauges removed and caps put in place until their requirement for service the
following day.

Flashback arresters will be provided on all regulators & torches of the acetylene welding sets. Insure
the special safety precautions are taken when unique welding operations are undertaken.

Only qualified personnel wearing appropriate PPEs will perform welding activities, checking the work
area for combustible material and possible flammable vapors as well as all cables and hoses to
protect from slag, heat, fire and sparks. Check cables and hoses for damage before starting work.

6.1.3 Excavations
 Excavations are the works made by the machinery and carrying hazards of collapsing of
the material excavated oxygen deficiency and hazardous gases. Open slope excavations in the
cut sections and cut-slopes cannot be evaluated in the categorization of excavation.
 Whenever there is likelihood of a collapse of the side of an excavation, the sides shall be
suitably sloped to a safe angle. Excavation reaching a depth of 1.2m or more shall be
sloped.

 Spoil materials will be set back at least 1m from the edge of the excavation. Heavy equipment
and vehicles will be kept as far away as practicable from the excavation.
 Suitable stop blocks will be used where vehicles or equipment tip material into an excavation.

 Persons will not be permitted to work in any place where they could be struck by any part
of mechanical equipment. Signs will be posted to warn personnel to keep out.
 Whenever there is likelihood of persons, equipment or vehicle falling into an excavation it will
be properly marked and barricaded.
 No mechanical excavation will be undertaken at a place if there is a possibility of
underground utilities.

6.1.4 Earthmoving
 Drivers or operators of mechanical plant and equipment will be trained, and should be
competent. Only authorized drivers and operators will be allowed to use equipment.
 Daily equipment check lists and preventative maintenance programs will be established to
ensure that all plant and equipment are systematically inspected, serviced, repaired and
maintained as necessary.
 Prior to earthworks commencing overhead and buried electrical cables and other pipelines will
be identified and precautions taken to prevent damage to these services.
 Non-essential personnel will be kept well away from the area of operation and trained banks
man will be provided at the borrow pit, dumping area and where necessary.
 Measures will be taken to prevent dust from impairing visibility by use of water spray on roads.
 Personnel working adjacent to haul roads will wear high visibility jackets and suitable warnings
will be displayed where people can see them.
 The possibility of vehicles coming into contact with overhead structures or power lines will
be prevented by erecting height gauges of the goal post type, constructed from non-
conducting material, distinctively marked with red and white stripes or bunting.
 Drivers will be instructed not to leave vehicles with their engine running and will not leave
the key over the vehicles when the driver leaves the vehicle.

Vehicles will be prevented from falling into excavations around borrow pit by:

 Clearly marking or securely fencing off the immediate excavation area,

 Providing a banks man to guide the vehicle,

 Vehicles will not be overloaded, and the loads will be evenly distributed, secured, and not
projecting beyond the sides or back of the vehicle. If some projection is unavoidable then the
load will be properly marked in order to ensure that the projection is clearly visible.

 Dumpers will not be allowed to travel with the body in a raised position unless inching
forward to discharge the load.
 Operators should be trained to recognize areas hazardous to dumping, such as soft or
uneven surfaces and inadequately compacted fill.
 Before dumping, operators should ensure that the tailgate is unlocked and that the vehicle is on
a reasonably level surface.
 Trucks should not dump when they are parked side by side with another vehicle. Dumping
operations should be spread out.

6.1.5 Dust Control


Excessive amounts of dust can cause eye and respiratory irritation, especially in dry conditions. In
general, dust and muck represent a nuisance to both workers and others in the vicinity. All traffic
routes in public areas near construction works should be kept clear of muck. To reduce the effects of
air-borne dust, water spraying is recommended at least two times per day.

6.1.6 Good Housekeeping


All personnel in the site show a safe housekeeping approach for the construction activities. Slips, trips
and falls are the main causes of injuries on construction sites, frequently as a result of litter and
debris. Therefore, on-site debris must be cleared from the job site at the end of each working day.

 Daily housekeeping will be carried out in all site facilities focusing on ground floor
offices, dormitory, toilets, and Canteen
 All construction consumables, scrap material and rubbish shall be placed in designated skips or
appropriate bins on or adjacent to the work site.
 All timber shall be free from protruding nails and piled neatly in a safe location, adjacent to
the immediate work site.
 Liquid spills will be cleared up immediately.
 Enclosed containers shall be used to dispose waste food from eating areas.
 Plastic, bottles, paper, etc. is to be collected in bins after coffee breaks, lunch and other
meal breaks.

 In addition to the daily washing of toilets by an appointed group, toilets shall be kept clear
of used rags, paper and other refuse that can lead to blockages.
 All Section Supervisors will be responsible for removal of their own rubbish and its disposal at
an approved site.

6.2 Potential Site Hazards and Precautions


The following hazards are identified as common to road construction sites and recommended
precautions are discussed below. It specifies the control measures required to reduce the
probability of the hazard arising and the consequences thereby

6.2.1 Slipping, falling and tripping


Some of the factors that can contribute to the risk of slips and falls are:

 Spill of hazardous Contaminants;

 floor surfaces and cleaning;

 environment, people and activity

 obstacles and related hazards


It is usually a combination of these factors that create the risk of a slip or trip. Slips usually occur
when there is a loss of grip between the shoe and the ground surface. This commonly occurs
when there is a contaminant between the shoe and the surface.

The proposed Mitigation Measures for risks associated with slips and falls include:

 The Contractor will implement good housekeeping practices, such as clear access ways
and prompt spills management;
 access to high risk areas will be limited and any changes in ground surface will be marked;
 The Contractor will provide adequate control measures for people working on steep batters;
 Adequate lighting must be provided in darkened areas to prevent people from falling, slipping or
tripping or being hit by projecting objects.
 Workers will be provided with suitable safety shoes.

6.2.2 Fire Protection
Many solids, liquids and gases can catch fire and burn. It only takes a source of ignition, which
may be a small flame or an electrical spark, together with air. Any outbreak of fire threatens the health
and safety of those on site and will be costly in damage and delay. It can also be a hazard to
people in surrounding properties.

Many fires can be avoided by careful planning and control of work activities. Good housekeeping and
site tidiness are important not only to prevent fire.

The Contractor shall take all appropriate measures for the prevention of and protection against fire on
the Site during the performance of the Works

Precautions to prevent fires


The following precautions should be taken to prevent fires:
 Smoking is prohibited on-site except in designated areas
 All open flame and spark producing equipment will be shut-down on site activities
 Firefighting equipment which complies with standard and specific requirements shall be
provided throughout and maintained in good condition (by a certified fire equipment provider),
especially at the offices, storage yard, fabrication yard, in the Engineer’s vehicles and in the
vehicles of his own staff.etc.

 Personnel shall be properly trained in the use of fire-fighting equipment.

 Fuel, oil and other combustible and flammable materials will be stored separately at designated
areas. Smoking will be allowed only in designated areas.

 Fire extinguishers will be inspected initially on first installation and then every month. Inspection
tag to be signed and dated.

 Extinguishers must be maintained in a fully charged and operable condition. The area in
front of fire extinguishers must be kept clear.

 The extinguisher will be made ready when doing hot work such as welding or using a disc
cutter that produces sparks;

 to minimize the risk of gas leaks and fires involving gas-fired plant:

 valves on gas cylinders must be closed when not in use;


 hoses shall be checked for wear and leaks regularly;

 prevent oil or grease coming into contact with oxygen cylinder valves;
 bitumen boilers shall be attended when alight;

 The site shall be checked at lunch time and at the end of the day to see that all plant and
equipment that could cause a fire is turned off. Stop hot working an hour before people go
home, as this will allow more time for smoldering fires to be identified; and

 Closed metal containers will be provided to collect rubbish and remove them from the site
regularly. Collect highly flammable waste such as solvent-soaked rags separately in closed fire-
resisting containers.

6.2.3 Traffic Safety and Precautions


The contractor is preparing Traffic Management Plan (TMP) of the road project and will submit it in the
third week of March 2020. Based on our past experience and documentation, the recommended
precautions to minimize traffic related hazards and risk that may arise at the project site include:

 All drivers will have the valid local driving license;

 The drivers shall be responsible for checking his vehicle's steering system, brakes, tires, seat
belts, horn, lighting, signal system, rear view mirror, wind shield wipers, oil level, radiator level,
exhaust system, gas tank cap and the seat belt of his vehicle;

 The Project employees and drivers shall enter/exit through designated access points for
passage of construction vehicles and personal. The parking location of all such vehicles
and equipment shall be outside the camp accommodation facilities and outside the
construction works area in the site for safety. The reverse parking will be applied for the
vehicles in the parking area for deploying them immediately in case of emergency.
 Vehicle Safety Inspection has to carry out by the HS officer in consultation with the
relevant personnel of the contractor.

 All vehicle accidents in the project site will be communicated immediately to the HS unit to
make the necessary communications to local traffic authorities and to the insurance company
to get the official accident report and the documents from them. Initially a verbal communication
will also be provided to the Employer representative depending on the severity of the
accident.

 Where construction activity requires the managing of traffic or pedestrians in nearby public
areas, trained flagmen (to slow down traffic) or trained stop-go men (to halt traffic) can be used

safety. Stop-go men and flagmen must wear high visibility vests and use approved stop-go
signs or flags. Where two stop-go men are required, they must be able to see each other
clearly or be able to communicate with each other by voice and where necessary with the
appointed works contact man, e.g. using two-way radio etc.
 All the incident/accidents in the Project will be reported to the HS officer to take the
following actions immediately. Actions need to be taken are investigating the incident/accident,
determining the corrective or preventive measure, reporting to the authorities and sharing the
incident/accident and its lessons learned Internally to the PM, project staff and to the
Management department.

6.2.4 Working at Elevated site Fall Protection


 The Contractor will provide appropriate fall protection for his employees working 2m or
more above the work surface;

 Employees shall be trained in safe use of fall protection systems before the equipment is used
as required;

 It’s strictly prohibited for anyone to ride a load suspended from a crane;

 Walking up or sliding down columns is strictly forbidden.

 Scaffolding will be used during the construction of bridges with the following precaution

 Scaffolds should never be erected or dismantled over people or busy pavements.


 Ensuring the scaffold is based on a firm, level foundation. The ground or foundation
should be capable of supporting the weight of the scaffold and any loads likely to be
placed on it.
 Before using any scaffold, make sure that it is safe and suitable for the intended job:
 ensuring platforms are fully boarded and wide enough for the work and for access
(usually at least 600 mm wide);
 checking that scaffold boards are properly supported and not overhanging excessively
(eg no more than four times the thickness of the board);
 ensuring there is safe access onto the work platforms, preferably from a staircase or
ladder tower;

6.2.5 Working at Night and Precautions
HS Technicians shall ensure that sufficient lighting is being provided during working at night. He/she
shall ensure that procedure is implemented at the planning stage of any night shift activities. The
following general aspects of work area lighting should be followed:

 All points of exit, pathways, and muster points shall be clearly illuminated and marked.

 Ladder access and egress shall be clearly illuminated.

 Tower lighting shall be located in a manner that illuminates all work areas.

 All workers shall wear fluorescent and/or retro reflective clothing including vests and gaiters

6.2.6 Lifting Operation and Precautions


The Contractor shall ensure all lifts are properly planned. The planning should include assessment of
risks associated with lifting, assembly activities and environmental conditions Including maximum wind
speed, ground condition, Rain and light.

Precautions and Planning for a safe lifting operation include:

 Before any piece of plant is selected and used to carry out an activity, it must be checked
for its suitability for the task (e.g. accessories available, reach capability, safe working load
(SWL), etc).
 Lifting appliances and lifting gear should never be used to lift beyond their stated safe working
load (SWL). This, as assessed by a competent person, is the maximum load that an item
of lifting equipment may raise, lower or suspend under the particular service conditions. All
lifting operations should be planned to ensure that they are carried out in a safe manner.
Generally a method statement should be prepared in advance of the operation.
 Lifting gear means any gear or cable by which a load can be attached to a lifting
appliance. It includes chain sling, rope sling, and hook, shackle or eye bolt. Before lifting gear
is used, it must be examined to check for safe working load (SWL) and so that defects, which
may reduce its capacity to function safely, are repaired. Lifting gear must be appropriately
certified prior to use.
 the Crane shall be well away from overhead power lines,

 Certified slinger and signaler must always be used where loads are lifted and safe
direction is given to operators of lifting appliances. The slinger and signaler directing a crane’s
movements should be easily identifiable to the crane driver (e.g. by the wearing of uniquely

identifiable high- visibility clothing, and/or the use of radio call signs).

 As a general rule, persons should not be working under an area where loads are being
lifted or within the working radius of the jib. People should be kept a safe distance from
working plant; barriers should be used where possible.

6.2.7 Electricity
Electrical equipment shall be designed, selected, operated and maintained in a way that is safe and
protect personnel from harm. The Contractor shall ensure that the following minimum
requirements are met;

 No bare wires are visible;

 The cable covering is not damaged and is free from cuts and abrasions (apart from light scuffing);

 The plug is in good condition, e.g. the casing is not cracked, the pins are not bent and the
key way is not blocked with loose material;

 There are no taped or other non-standard joints in the cable;

 The outer covering (sheath) of the cable is gripped where it enters the plug or the equipment.
The colored insulation of the internal wires should not be visible;

 The equipment outer casing is not damaged and all screws are in place;

 There are no overheating or burn marks on the plug, cable or the equipment;

Overhead power lines


Contact with overhead power lines is a regular cause of death and injury. Any work near electric
distribution cables must be carefully planned to avoid accidental contact. The most common
operations leading to contact with overhead lines are:

 operating cranes and other lifting equipment;

 raising the body or inclined container of tipper trucks/dump truck/ and loaders;

 operating excavators and other earth-moving equipment;

 handling long items such as scaffold tubes, metal roof sheets, ladders etc;

Where possible all work likely to lead to contact with the overhead line will be done in an area well
clear of the line itself. The contractor will ensure that, if plant is working in the vicinity of an overhead
power line, the distance between the plant and the overhead line should be at least:

 15 m (plus the length of the jib) if the lines are suspended from steel towers; or

7 Emergency Response Plan
7.1 General
In the event of emergency situation at construction site, the procedures noted in this section are to be
immediately followed. Emergency conditions considered to exist are if:
 Any member of the field crew is involved in an accident or experiences adverse effects or
symptoms of exposure while on the site;
 A condition is discovered that suggests a situation more hazardous than originally
expected; and/or,
 A spill of fuel or other hazardous material.

7.2 Emergency Procedures


7.2.1 Camp site Emergency Procedure
 As soon as fire, bomb or other threat noticed, immediately evacuate the local area and
notify others on site to do the same.
 Do not attempt to fight fire unless properly trained and appropriate fire-extinguishing equipment
available.
 Encourage all persons on site to remain calm and orderly.
 Shut down any plant or equipment still operating.
 Do not go back for tools, equipment or personal belongings. Do not attempt to move
vehicles unless advised by emergency services.
 If escaping through a smoke filled area, keep low to the floor [more oxygen] and move
cautiously towards safe areas.
 If trapped on a roof or higher level of the building, go to an outer window or space where a
door can be shut and exit via scaffolding or attract attention from the window.

 Once clear of the area and building – assemble at the front of the property

7.2.2 Procedure: Chemical Exposure


If a site worker shows symptoms of chemical exposure, the procedures outlined below should be
followed.
 Precautions should be taken to avoid exposure of other crew members to the chemical;
 If the chemical is on the worker’s clothing, the chemical should be neutralized or removed
if it is safe to do so;

 If the chemical has contacted the skin, the skin should be washed with liberal amounts of water;
 In case of eye contact, an emergency eye wash station should be used. Eyes should be
washed for at least 15 minutes; and,
 All chemical exposure incidents must be reported in writing to the HS officer.

7.2.3 Procedure: Personal Injury


First aid kits will be maintained at the project site at all times. The following procedures should be
followed if a personal injury occurs at the site:
 Another team member should signal the HS officer that a personal injury has occurred;
 A field team member trained in first aid may provide initial emergency treatment to the
injured individual if appropriate;
 If the status demands so, the ill or injured person will be transported by ambulance to the
nearest hospital depending on the type of medical emergency. And when necessary, the
Contractor will coordinate readmission of the person to another hospital facility.
 The HS officer will prepare the Incident Report Form. Follow up actions should be taken to
correct the situation that caused the accident;
 Any near miss incident, property damage, first aid, or medical treatment received must
also be reported.
 A site injury summary will be maintained listing all medical treatment (first aid) given at the site.

7.2.4 Procedure for Shock


 Keep victims lying down and quiet

 Keep victim covered with a blanket or something similar to keep body temperature at normal, 37’C

 Keep victim head lower than feet to promote blood supply to head and chest

7.2.5 Procedures: Major Fire or Explosion


In the event of a major fire or explosion, the following procedures will be instituted:

 Notify the nearest or local fire hazard prevention office/if any/ immediately as necessary;

 Isolate the area;

 Stay upwind of any fire;

 Keep the area surrounding the incident clear after the incident occurs; and,

 The HS/ site engineer should complete an Incident Report Form.

7.2.6 Procedures: Electrical and flammable Fluids
In the event of an emergency involving electrical, flammable/explosive of release of dangerous
toxic materials such as gases, fluids or powders,

 All work will cease immediately, personnel will be removed from the site to a safe distance
of at least 100 meters. and follow the procedures

 In the event of any life threatening emergency, the procedure is to provide first aid if
required and safe to do so, and call emergency contact persons for the Ambulance
immediately and the casualty is transported to medical care.

 In the event of non-life threatening injury or illness, first aid is provided, the HS/ site
Engineer/ Project Manager is informed and the casualty is transported to medical care.

 In the event of any threatening event concerning utilities, work in the immediate area will
cease immediately and the utilities contacted.

7.2.7 Procedures: Extreme weather (Winds/Rain Fall)


 In the event of high winds, heavy rainfall causing hazards such as flying debris, poor
visibility, unstable footing, the site supervisor, after having evaluated the hazard will decide on
the course of action which may include temporary stoppage of work at the site, providing
shelter for exposed workers and stoppage of use of heavy equipment at the site.

 The HS/ site engineer will decide if the weather condition will present an ongoing hazard
to workers, in which case he/she can secure the site and conclude work for the specified work
period. Work will resume after a site inspection is performed to ensure that the work
conditions are safe and work can resume.

7.2.8 Procedures: Electrical Storms


 In the event of electrical storms, the site supervisor, after having evaluated the hazard will
decide on the course of action which will include temporary stoppage of work site,
providing shelter for exposed workers and stoppage of use of heavy equipment at the site.

 The HS/ site engineer will decide if the weather will present an ongoing hazard to workers, in
which case he/she can secure the site and conclude work for the specified work period. Work
will resume after a site inspection is performed to ensure that the work conditions are safe
and work can resume.

7.2.9 Procedures: Trench and Excavation collapse
 Create a safe zone.

 Begin dirt removal, operating from a safe zone (buckets, small shovels, by hand).

 Continue extending safe zone into collapse zone.

 Continue dirt removal.

 Uncover victim to below the diaphragm.

 Begin patient assessment if possible

 Begin ventilation if possible.

 Completely uncover the victim.

 Proper patient packaging.

 Remove the victim from the trench (vertical haul, horizontal haul).

 In the event of any life threatening emergency, the procedure is to provide first aid if
required and safe to do so, and call emergency contact persons for the Ambulance
immediately and the casualty is transported to medical care.

 In the event of non-life threatening injury or illness, first aid is provided, the HS/
siteEngineer/ Project Manager is informed and the casualty is transported to medical care.

7.2.10 Spill Response


 The execution of suitable arrangements for ensuring safety and absence of risks to health
in connection with the use, handling, storage, transport and disposal of particles and
substances. Transportation of any material by the Contractor shall be in suitable vehicles which
when loaded does not cause spillage. All loads shall be suitably secured. Any vehicles
failing to comply with these requirements or any of the local traffic regulations and laws shall
be removed from the Site.

 Any spill shall be reported immediately to the HS/site Engineer. He/she will then order
mitigation measures and report to the Project Manager.

 Spill control equipment, such as sorbent materials and booms, will be located on-site.

 All materials used for the cleanup of spills will be containerized and labeled separately from
other wastes.

 HS/ Site Engineer will determine if additional spill response measures are warranted.

 Any impacted soil will be removed and stockpiled or drummed for offsite disposal at a
permitted facility.

7.2.11 Evacuation Procedures


The HS will initiate evacuation procedures by signaling to leave the work area. All personnel in
the work area should immediately evacuate the area and meet in the pre-determined evacuation area
so that a head count can be performed. All personnel suspected of being in or near the work area
should be accounted for and their whereabouts determined immediately. The HS/ site Engineer will
provide further instructions as to whether it is safe to return to the work area or not.

In case of accident on working site, construction material production and processing sites as well as
camps the following emergency procedures will be followed:

 DO NOT move the injured person / persons unless they or your self are exposed to
immediate danger.

 Contact the Forman/Site Engineer/First Aider State “Emergency, Emergency, Emergency”.


Wait for the representative to respond.

 the Forman/Site Engineer/First Aider will inform the Site Engineer or HS 0fficer and
medical personnel regarding the accident;

 All personnel not involved in the emergency are to maintain radio silence during the emergency.

 Advise the representative who you are, details and location of the incident or the number of
people injured and what injuries they have and whether you are able to help the injured
person(s).

 Site Engineer or HS officer will assess the accident, e.g. if ambulance and medical team
required to move/transport casualty to the clinic and/or the nearest hospital.

 Victims incurring minor injuries will be brought to the clinic by service vehicles readily available
at the job site

 Comfort and reassure the injured person(s) where possible, until help arrives.

 Alert others in the area and secure the area to the best of your ability to prevent further
damage or injury.

 If directed by the HS Officer, evacuate the site as per the Evacuation Procedure. All personnel
are to remain at the assembly area until otherwise informed by the Emergency Response

 If unable to contact the Forman/Site Engineer/First Aider, call the HS officer and other Key
MELCON Personnel and describe the emergency services required by stating your name,
location/chain-age and nature of injury/accident and number of injured persons.

 Upon notification the HS officer, all construction crew will respond to the emergency scene
equipped with First aid kits which will be available in all camps, garages and main
construction sites.

7.2.12 List of Contact Information


Typical emergency contact list MELCON staffs will be placed in the camp and vehicles.

8 Monitoring and Review Process


The project health and safety officer in collaboration with environmental advisor will prepare monthly
report in the form of checklist to be in the environmental and safeguard performance report to be
submitted to the employer’s representative. The report/checklist should include status of the
following HS monitoring indicators:

 Establishment of first aid clinic equipped with medical equipment’s and availability of first aid kit
at construction sites;
 Presence of Health and Safety awareness creation posters at camp sites;
 Provision of PPE to workers and number of workers with PPE at work sites, workshops;
 Erection of protection fence at construction sites;
 Fire extinguishers are installed at offices, fuel station and workshop area;
 Presence of security guard and warning signs at camp and material sites;

The Health and safety management plan will be subjected for a review as part of a continuous
improvement approach to ensure its continuing suitability, adequacy and effectiveness. The
management review process should ensure sufficient information is gathered over the term of the
project to allow management to undertake an effective review up on consultation with ER’s.
