3DCT OF THE FACE AND NECK WITH CONTRAST AND LIMITED MRI

Clinical profile: Right maxillary ameloblastoma, post-operative status.

Pre and post-contrast CT of the face and neck, 3D CT skull face, has been performed with
limited non contrast MRI.

The study reveals evidence of previous right maxillary region surgery.


A large multiseptated and multiloculated lesion with ‘soap-bubble’ appearance is seen at the
right maxillary sinus antral region.The lesion measures 5.5 x 4.5 x 5.0 cm in maximal AP,
transverse and craniocaudal dimensions. The lesion shows multiple speckled calcific foci /
hyperdense areas with heterogenous scattered areas of enhancement and internal septae.

It shows a complex signal on MR sequences with restricted diffusion, hypointense on ADC


maps with reduced ADC values.

It is seen extending to :
- Right nasal cavityy with mass effect on right sided nasal structures.
- Right premaxillary soft tissues and right retroantral region through bony wall defects.
- Right nasopharynx, infratemporal fossa and right masticator space.
- Uplifting floor of right orbit without any intra-orbital extension.

Right maxillary alveolar ridge also shows osteolytic foci.Other visualized bones are normal.

Right pterygopalatine fossa is intact.Palatine foramina are normal.

Retained secretions are seen within the right maxillary antrum surrounding the mass on
MRI.

The rest of the visualised neck looks normal.Bilateral multilevel cervical lymphadenopathy
is noted at level IA,IB,II and III / V. The nodes are oval in shape with intact fatty hilum and
range in size from 7-14.2 mm.

Impression:

Above CT / MRI findings are suggestive of a large recurrent ameloblastoma with extension
into adjacent regions as described above.
Bilateral multilevel cervical lymphadenopathy is noted.
Suggest: HPE correlation and followup.
