1. Purpose
The purpose of this plan is giving Criteria for determining the feasibility of providing safe access and
fall protection for scaffold erectors and dismantlers.
2. Scope
Covers all scaffolds (erection and dismantling) in workplaces.
3. Definitions
- Scaffold: Any temporary elevated or suspended platform and its supporting structure used for
supporting employees or materials or both, except this term does not include crane or derrick
suspended personnel platforms.

- Anchorage: Safe points of anchorage for lifelines or lanyards, which are part of a personal fall arrest
system, including fixed, substantial structural members. Anchorage points shall be Fixed and able to
support a load of at least 2,260 kilograms (5,000 pounds).

- Base Plate: A steel plate providing a flat bearing surface with a spigot or screw jack for
Distributing the load from posts (standards). It has an integral spigot and fixing holes for use
With sills.

- Bearer (Transom): A horizontal transverse scaffold member that may support platform
units, and which is supported by at least two runners (ledgers) or connected directly to at least two
posts (standards).

- Board Bearer (Intermediate Transom): A horizontal transverse scaffold tube upon which
the scaffold platform partially rests. Board bearers are supported by runners (ledgers) and
are not installed near a transverse line of posts (standards).

- Body Belt (Safety Belt): A strap with means both for securing it about the waist and for
attaching it (with a lanyard) to a lifeline or anchorage. Used to provide personnel positioning
limits against a fall (i.e., keep wearer away from edge of a roof).

- Box Tie: An assembly of tubes and couplers forming a frame around a column or other part
of a building to provide an anchor point for scaffold tie tubes. Also known as a column box tie.

- Brace: A rigid connection of scaffold tubing that holds one scaffold member in a fixed position with
respect to another member to give the scaffold rigidity. Types of Bracing (Cross Bracing, Diagonal
Brace and Plan Bracing)

- Caster (Castor): A pivoting wheel, containing a wheel lock, which is attached to a vertical
post (standard) of a mobile scaffold to allow for manual movement of the scaffold.

- Coupler (Fitting, Clamp): A component or device used to fix scaffold tubes together.

Types of couplers include:


• Adjustable (Swivel) Coupler: A device used for connecting two tubes together at an angle
other than 90
• Bearer (Single or Putlog) Coupler: A device used for fixing a bearer (transom or putlog) to a
runner (ledger), or to connect a tube used only as a guardrail to a post (Standard).
• Check (Safety) Coupler: A Right-angle coupler added to a loaded joint on an under hung
scaffold to give supplementary security against slip to the coupler carrying the load.
• End-to-End (Sleeve) Coupler: A device used for externally joining two (2) scaffold tubes co-
axially end to end. The steel divider is located centrally to ensure equal insertion of each tube.
• Girder Coupler: A device used for connecting a scaffold tube to a steel wide flange beam (I-
beam). Girder couplers shall only be used in pairs, one on each opposite
Flange. Also called an “SK” (Steel-Klamp).
• Right-Angle: (Double, 90 Degree, Load Bearing, or Standard) Coupler A load bearing coupler
used for connecting two tubes together at right angles. It is a critical component in the scaffold
structure and must resist both slip and distortion.
4. Types of scaffold according duties:
4.1- Light-duty: Scaffold designed and constructed to carry the weight of workmen only,
with no material storage other than the weight of tools. Commonly used for
Inspection, painting, access and light cleaning. Uniformly distributed maximum
intended load is 120 kg/m 2
4.2- Medium-duty: Scaffold designed and constructed to carry the weight of light
materials, tools and workmen. Uniformly distributed maximum intended load is 240 kg/m 2
4.3- Special-duty: Scaffold specially designed and constructed to carry maximum
intended loads greater than 240 kg/m 2

6. Procedures
6.1- Scaffolding Hazards:
• Collapse of Scaffolding.
• Falling from height and Falling object.
• manual handling.
• Slip & Trip hazards.
• Pinch point hazards / Sharp edges
• Opening without guardrail.
• Scaffold erection during storm or high winds, raining and poor visibility.
• Blocking emergency access and walkways
6.2 Safe Scaffold Erection and dismantling:
6.2.1- Scaffolding Erection basis.
Safe scaffold erection and use is important in minimizing and controlling the hazards associated
with their use. Scaffold work practices and rules should be based on:
 Sound design and material inspection before use(by inspector and supervisor)
 Selecting the right scaffold for the job
 Assigning personnel
 Fall protection
 Guidelines for proper erection
 Guidelines for use
 Inspections
 Maintenance and storage

6.2.2-Erection procedure:
 Area of work to be barricaded.
 Daily on job TBT instructed by job performer and HSE to all employee identify job hazard with
control of it.
 Inspect working tools and saving from falling.
 Base out scaffold and erect base lift and first lift.
 prevent unauthorized access to scaffold area.
 one scaffolder holds standards, while another places transoms and ledgers.
 adjust screw jacks to level the scaffold.
 erect transoms and ledgers for the first lift.
 erect planks from below for the first lift.
 Complete first lift.
 install access stairway or ladder to platform for the first lift.
 install edge protection comprising guardrail, mid-rail and toe board to first lift.
 Erect next lift from a platform erected firstly.
 standing on the working platform place planks within the transoms to form the above working
platform.

 install ladder or stairway access to working platform above.


 access the above working platform
 install toe boards (or other systems to prevent objects falling).

6.2.3-Dismantling procedure:
 Area of work to be barricaded.
 Daily on job TBT instructed by job performer and HSE to all employee identify job hazard with
control of it.
 Inspect working tools and saving from falling.
 Start dismantling from an end (higher lift to down).
 move down to the platform below
 dismantle top platform planks
 working from an erection platform on the fully planked platform, dismantle guardrails, mid-
rails, ledgers, transoms and standards from the level above
 remove planks (ensure at least two planks remain) to enable the dismantled materials to be
passed down to the next level (where they are to be temporarily stacked or removed from the
scaffold)
 if removing dismantled materials progressively down through the scaffold, ensure that the
gaps created are not directly below each other and that planks are replaced when a gap is not
being used to lower materials with no storing martial on scaffold platform so use sufficient
scaffolder in handling and lowering material with care.
 Remove ties and bracing (with care non- swing for scaffold) progressively as the scaffold is
dismantled.
 progressively repeat this procedure until dismantling can be completed from the ground

7. General Safety Rules of Scaffolding:


7.1-General scaffolding requirements:
o Scaffolding shall only be erected, dismantled or modified by qualified Persons under
supervision of competent scaff.tech.
o Erecting of scaffold shall not be signed off as complete until the Scaffold has been inspected
and approved for use by a Scaffolding inspector& Scaffolding tag "SCAF-TAG" indicating
acceptance.
o Uprights and ledger shall not be used. Metal parts used for scaffolds shall be in good
condition and free from corrosion.
o All poles, planks and general materials, used for scaffoldings, shall be kept in good Condition
and be inspected by a competent person (inspector and supervisor).
o A scaffold shall be erected only by men knowledgeable in the job, working under the
immediate supervision of an experienced foreman, who knows the purpose of the scaffold
and how it should be constructed to carry the loads which will be placed upon it.
o Scaffolds shall be securely supported or suspended and where necessary braced to ensure
stability. Unless constructed as an independent scaffold, it shall be rigidly connected to the
building or structure.
o In the case of partially erected or dismantled scaffolds still capable of being used, access there
to should be effectively blocked and prominent warning notices in English/Arabic languages
shall be posted. The "SCAF-TAG" is to be removed.
o All platforms, scaffolds and other workplaces, from which persons may fall more than 2 m (6
ft. 6 inches) shall have edge protection which consist of an upper rail not less than one meter
(3 ft. 3 inches) in height above the walkway and have at least one intermediate rail.
o Support its own weight and 4 x maximum intended load
o Suspension rope and connecting hardware support 6 x maximum intended load
o Vertical toe boards shall be fitted to all scaffolding.
REQUIREMENTS FOR BOARDS AND PLANKS
o a) Boards of 38 mm (1.5 inch) minimum thickness shall be used. These shall be at least 225
mm wide.
o b) The spacing of board supports shall depend on the thickness of the boards used and the
load to be carried. There shall be at least three supports. Support for 38 mm (1.5 inch) boards
shall not be more than 1.5 m (4.9 feet) apart. Support for 51 mm (2 inch) boards shall not be
more than 2.5 m (8 feet 6 inches) apart. All boards shall be supported at the ends.
o Boards shall be end-butted and close boarded throughout. Overhanging of boards Platforms
10’ and less to extend at least 6” but not more than 12” past support Platforms greater
than10’nor more than 18”.
o Overlap platforms not be less than 12” only over supports unless restrained to prevent
movement.
o Platform slopes shall not exceed 5.08cm (2inch) in 10 feet.
o No more than 1” gap between adjacent units and platform and uprights
o Platform and walkways at least 18” wide

7.2 Design Load:


o Scaffolds and their components shall can support without failure at least four times the
maximum intended load. Scaffolds shall not be loaded more than the working load for which
they are intended
7.3 Capacity:
o 4 times the intended load + scaffold weight
o Planks must not deflect more than 1/60 of span when loaded (2’’ in 10 feet)

7.4 Access:
o Safe means of access such as walkways, stairs or fixed ladders shall be provided to all working
platforms.
o While working on long pipe racks, workers shall be no more than 7m (21ft) away from access
ladders at any moment, so access/egress points will be available each 14m (42ft)
o No access by cross braces
o When using ladders, bottom rung no more than 24 inches high
-The clearance between working platforms and power lines shall be as follows:
o Uninsulated lines < 50KV -----3m (10ft)
o Uninsulated lines > 50KV ------3m (10ft) + 10.16 cm (4inch) for each KV over 50K

8. Fall Protection:
A. PFAS consist of anchorage, connectors, deceleration device.
B. PFAS must be rigged so that an employee can neither fall more than neither 6 feet nor
contact any lower level. Bring an employee to a complete stop and limit maximum
deceleration distance and employee travels to 3.5 feet.
C. PFAS must be inspected prior to each use for wear damage, and other deterioration. Defective
components must be removed from service. Dee-rings must be made of drop forged, pressed
or formed steel, or made of equivalent material with a minimal tensile strength of 5,000
pounds
D. Snap hooks shall be a locking type only snap hook designed and used to prevent
disengagement of the snap hook by the contact of the snap hook keeper by the connected
member.
E. Self-retracting lifelines and lanyards which do not limit free fall distances to 2 feet or less rip
stitch lanyards, and tearing and deforming lanyards shall can sustain a minimum tensile load
of 5,000 pounds applied to the device with lanyard in the fully extended position.
F. Supervision shall ensure continuous monitoring of employees wearing a personal fall arrest
System so that prompt assistance is possible in the event of a fall.

9. Protection from Falling Objects and manual handling:

o When toe boards are used with the guardrail system as protection from falling objects, they
must be erected along the edges of the overhead walking/working surface for a distance
sufficient to protect persons walking below. Toe boards shall be at least 4inches in height and
withstand 50 pounds of force in a downward and outward direction.
o When roofing materials and equipment shall not be stored within 6 feet of a roof edge unless
guardrails are erected at the edge and materials piled, grouped or stacked near a roof edge
must be stable and self-supporting.
o Safe handling for material during erection and dismantle from falling by using barricaded area
of work prohibited to any person under the load lowered or raised with good communication
with scaffolder during handling in each lift even hand by hand or other lifting way such as
robe or hoisting inspected before job.
o No person receives or give other material even load is secured from falling with care method
and cleared area not stored on lift
o Continues housekeeping in area of work and transfer to storing area.

10. Training and Competency


Only the trained authorized scaffolders are authorized to erect and dismantling of scaffoldings.
The training shall include and cover the following topics:
 Hazards of working at height
 Fall protection techniques
 Types and classifications of scaffolding
 Right selection of scaffolding material
 Emergency response planning

‫خطوات تركيب السقاالت‬


‫• خطوات العمل ‪:‬‬

‫‪ -1‬نقل المعدات المستخدمة في انشاء السقالة الي مكان العمل وذلك بعد فحصها وفحص عدد‬
‫التركيب المستخدمة‪.‬‬
‫‪ -2‬احاطة مكان انشاء السقالة بشريط تحذير‪.‬‬
‫‪ -3‬يتم تثبيت قاعدة السقالة بحيث ( يجب توفير ألواح مناسبة أسفل أرجل السقاالت )‬
‫وأن تمتد مسافة ال تقل عن ‪ 9‬بوصة من كل جانب(‪22.5‬سم)‬
‫‪ -4‬يتم تركيب القوائم الرأسية علي القواعد التي تم تثبيتها‪.‬‬
‫‪ -5‬يتم تركيب العوارض األفقية علي القوائم الرأسية في األماكن المخصصة لها والتأكد من‬
‫احكام تثبيتها‪.‬‬
‫‪ -6‬يتم وضع )‪. (transom‬‬
‫‪ -7‬يتم رص األلواح الخشبية علي )‪ (transom‬بحيث ال تزيد المسافة بين كل لوحين عن‬
‫‪2.5‬سم ‪.‬‬
‫‪ -8‬يتم تثبيت حواجز منع السقوط )‪ (mid rail‬وذلك علي ارتفاع ‪55‬سم ‪(hand rail) ,‬‬
‫وذلك علي ارتفاع ‪110‬سم ‪.‬‬
‫‪ -9‬يتم تثبيت حاجز لمنع سقوط أو انزالق األدوات)‪ (toe board‬بحيث يكون أقل ارتفاع له‬
‫‪10‬سم‪.‬‬
‫‪ -10‬اكمال المنصة مع مع عمل النهايز والتعصيبات الالزمة ‪.‬‬
‫‪ -11‬يتم تركيب السلم للمنصة األولي بحيث يميل بمقدار ‪. 1:4‬‬
‫‪ -12‬يتم وضع )‪ (red scaffolding status‬وتثبيته علي السلم‪.‬‬
‫‪ -13‬يتم تكرار الخطوات السابقة طبقا لعدد المنصات المطلوبة بتركيب المنصة التالية من على‬
‫المنصة االولى بتركيب العوارض والترانسوم وتطريح الخشب وعمل سلم للصعود‬
‫واكمالها كما بالسابق‪.‬‬

‫‪ -3‬يتم البدء بالفك بداية من الدور األعلي ‪.‬‬
‫‪ -4‬يتم البدء بفك حواجز االمان من السقوط اوال وواقى القدم للمنصة العليا‪.‬‬
‫‪ -5‬فك الواح وترانسوم المنصة من على المنصة األسفل ومن ثم سلم المنصة العيا‪.‬‬
‫‪ -6‬فك العوارض والقوائم‪.‬‬
‫‪ -7‬فك النهايز والتعصيب ‪.‬‬
‫‪ -8‬تنزيل العدد التى تم فكها لالرض ‪.‬‬
‫‪ -9‬تكرار العملية طبقا لعدد األدوار المطلوبة‬

JOB DESCRIPTION FOR THE SCAFFOLD CREW

1- Scaffold Supervisors
- Inspection of the work area to detect hazards and implement necessary corrective
action.
- Make spot checks of assigned tools and scaffolding on which personnel are at work.
- inspect tools and scaffolds before use.
- Maintain an effective line of communication of safety matters with the workers and give
daily on job TBT
- Assist with developing and communicating safe job procedures for unusual or hazardous
operations.
- Observe the crew to see whether they are sufficiently experienced. Also, if they are alert
and in good physical condition.
- Close monitoring to all worker perform task to be working in safe manner with
continuous communication

2- Scaffold tech. (scaffolder)


- Has been fit tested and trained in scaffold skills.
- Adhere to PETROMAINT and UGDC safety rules
- Comply with hazard analysis and safety requirements established on work permits.
- Sufficient skills in erection and dismantling of scaffolding and working at height in safely
method.

3- Scaffold helper
- Adhere to PETROMAINT and UGDC safety rules
- handling scaffold tools safely to work site.
-have knowledge in scaffold tools and safe manual handling on bottom (not working at
height)
- housekeeping and maintain work site to avoid trip hazards.
