What happened?
A crew of three (3) subcontractor employees were in the process
of performing housekeeping works at the rooftop of the Main
Administration Building.

Two of the workers worked in tandem to remove plywood sheets

that were stacked together in one area and were transferring them
to a different location. As they lifted the last sheet of plywood that
had three (3) doka formwork “H” beams secured to the bottom of
it, workers failed to notice that they had in-fact removed the hole View of the main admin building
cover. Injured party stepped forward and into the floor opening,
through the concrete shaft and fell 15m below to the concrete
surface at the ground level.

What went wrong?

• Workers failed to recognize that they removed the floor
opening cover while performing housekeeping works.
• The crew were not aware of the floor opening at the
rooftop.
• The shaft cover had no warning signs posted to warn the
people of the fall hazard.
• There was no physical barrier installed to isolate the Bird view of the shaft
hazard.

Outcomes
The IP suffered serious injury resulting in lost time (LTI). Under slightly different circumstances the incident could
have resulted in fatality.

What you should do? Verify implementation of below at your Project locations:

ALL AREAS WHERE FALL HAZARDS MAY

FRONT-LINE SUPERVISORS MUST ENSURE BE PRESENT MUST BE IDENTIFIED AND,
THAT ALL HAZARDS ARE COMMUNICATED
! TO CONCERNED WORKERS AT SITE PRIOR
TO START OF WORK
! WHERE REQUIRED, PROVIDED WITH
ADEQUATE RAILING SYSTEM IN THE CASE
OF FLOOR OPENINGS

ENSURE THAT ALL FLOOR OPENINGS

AT SITE ARE ADEQUATELY SECURED
! AND COVERED WITH SIGNS POSTED TO
ALERT PEOPLE OF A FALL HAZARDS
