PREFACE
This Petrofac Health and Safety, Security, Environment and Integrity Assurance Management
(HSSEIA) Standard describes the expectations and performance requirements to be met with
regard to risks to health and safety, threats to security, aspects of our environmental impact and
assurance of the integrity of our assets. This document sets out the approach taken to
implement the Petrofac Limited Policies for HSSEIA.
This standard has been approved by the Petrofac Senior Vice President HSSEIA and is
mandatory for all Petrofac Limited subsidiaries, Joint Ventures where Petrofac have control and

contractor activities insofar as these are referenced in the contract.

Each Petrofac division and business unit is required to adopt this standard as a mandatory
requirement. Guidelines, tools and supporting information relating to this standard are provided.

2.0 APPLICABILITY

4.1

Golden Rules of Safety - Lifting Operations
Business units shall adopt the Golden Rules of Safety to ensure the safe

conduct of lifting operations.
Lifts utilising cranes, hoists or other mechanical lifting devices will start only
when:

• an assessment of the lift has been completed and the lift method and equipment
has been determined by a competent person
• operators of powered, lifting devices are trained and certified for that equipment

4.2 Planning
Prior to the commencement of any lifting activity and as part of the lift planning process,
it is essential to conduct a suitable risk assessment. This risk assessment shall
categorise the lifting operations according to their level of risk and complexity. It shall
also consider all the options available to ensure that people and assets are not exposed
to risks as a result of the lifting activity.
During the planning, design and construction of buildings, structures, plant and

equipment, considerations shall be given to:
• completion of a suitable and sufficient risk assessment

• the movement of materials and equipment around the site to ensure that there
are no lifts over people or critical process equipment
• the location of set down areas and safe access for the lifting equipment

• the position of possible underground/buried services (see Ground Disturbance
Standard, PL-STAN-HSSEIA-002)
• stability of site/ground conditions (see Ground Disturbance Standard, PL-STAN-
HSSEIA-002) d
4.2.3 Lift plans

A lift plan shall be developed for all lifting activities and as part of the overall control of
work process. The plan shall be supported by a suitable risk assessment which shall
identify the hazards and risks associated with the lift. The plan shall be reviewed and

approved by the lifting competent person. The details required in the plan shall be
related to the risk and complexity of the lift. A generic lift plan which shall be supported
by an on-site risk assessment and team briefing may be developed for routine lifts.
Generic lift plans shall be formally reviewed and re-issued periodically.

• the type and number of personnel required for the lift, their specific roles and
competency levels
• equipment required for the lift and their certification checks
• the lift pick-up and set down positions and any constraints such as space and

stacking
• step-by-step instructions for the lift
• communication methods to be used during the lift
• contingency and rescue plans
• any simultaneous, conflicting or nearby operations or works including other lifting
operations
• any applicable permit to work
• any restrictions on the lift such as weather conditions, visibility conditions, sea
state, proximity to overhead power lines/cables including setting minimum
clearance distance, etc.
The lift plan for all critical lifts shall be prepared and reviewed by a qualified engineer.
The plan shall set the limitations applicable to the lift based on the critical lift criteria.
These shall include but not limited to:
• weight of the equipment to be lifted compared to the rated lift capacity
• the swing area of the lift
• the soil bearing capacity of the lift location
• the overall risk, difficulty or complexity of the lift
• toxicity of the item or product being lifted

• proximity of the lift to pipelines, electric cables, other installations or facilities

All lifting operations that require a permit to work shall be identified and documented in
accordance with the Permit to Work Standard (PL-STAN-HSSEIA-007). The approved
lift plan shall be attached to the permit to work certificate.
Any deviations or changes to an approved lift plan shall be subject to a fresh review and

approval by the lifting competent person.

Works involving lifting operations shall be carried out as part of the overall control of
work process. As such, steps involved may already be present but shall be part of an
independent system that meets the requirements of the Control of Work Standard (PL-
STAN-HSSEIA-013).
Effective controls shall be in place for all lifting operations. The controls shall follow the
hierarchy of risk controls and shall include the appointment of one competent person
from the work team as the person in charge of the lifting operation who shall, among
others:

• review the lift plan and ensure that the required controls are in place
• ensure that the lift is carried out in accordance with the lift plan

• suspend the lift if changes or conditions (e.g. weather) occur that would cause a
deviation from the lift plan
• ensure that the lift team has understood and tested the communication methods

to be used for the lift before the lifting activity begins
• ensure that the lifting equipment is inspected and appropriate for use
• ensure that all personnel involved in the lifting operations are competent, are

aware of and clearly understood their allocated tasks
• ensure that personnel are kept clear of overhead loads by using appropriate

barriers and segregations
• supervise and monitor the lifting operations and the performance of all personnel
involved to ensure that adequate performance standards are maintained

The load to be lifted (including the lifting accessories) shall be confirmed as within the
rated capacity of the lifting equipment and attached by means of suitable lifting
accessories.
As necessary, loads shall be properly attached at their centre of gravity and kept
on
reasonably level. Where the centre of gravity of the load is not easily identified or
marked, trial/test lifts shall be used to assist in identifying it. Taglines shall be used to
assist in stabilising slung loads.
The lifting appliance operator shall not leave the operating controls while the load is
nc
suspended.
No one shall be positioned under a suspended load or between a suspended/lifted load
and fixed objects. People shall have an escape route in case of unexpected movement
of the load or equipment.

A site-specific emergency response plan shall be in place in all locations where lifting
operations will take place in line with the Emergency Response and Crisis Management
Standard (PL-STAN-HSSEIA-010). The lifting appliance shall be fitted with appropriate
safety devices such as emergency warning, emergency shutdown devices and limit
switches. These shall be tested and confirmed to be operational before commencement
of lifting activities.
Lifting activities shall be suspended in the event of any of the following:
• the scope of work changes
• an unexpected change in circumstance, e.g. weather conditions
•

any deviation from the approved lift plan
Whenever lifting activities are suspended, the risk assessment and lift plan shall be

reviewed, with any necessary remedial actions taken and re-approved by the lifting
competent person before re-starting.
4.3.3 Communication

A tool box talk/briefing which will be signed by all participants shall be conducted prior to
the commencement of each day’s lifting operation. The briefing shall identify:
• the task being performed d
• who is doing what tasks and the hazards associated with the tasks
•

any hazardous positions/location that needs to be avoided
• other operations taking place
• communication method to be used e.g. verbal, hand signal or radio method

The briefing shall be repeated before a team member takes a different task or when new
personnel are added to the team.
Communications related to lifting activities including those between the lifting appliance
operator and the banksman/signal person shall be clear, direct and definite. The

The banksman/signal person shall be located in such a position that they:
• can be clearly seen by the lifting appliance operator

• can maintain visual contact with the load and load handler(s)
• see potential lifting appliance boom collision points
The banksman/signal person shall not undertake more than one task at a time, for
instance the banksman/signal person shall not be a rigger and a banksman/signal
person at the same time.
The banksman/signal person shall be easily identifiable from other workers participating
in the lift.
It is essential to facilitate good communication during lifting operations therefore
personnel involved in lifting activities shall not be distracted.
Confirmation that the work has been completed shall be communicated to all affected
persons.
4.4 Training
Personnel shall be suitably trained and competent prior to planning or undertaking any
lifting operations. The training shall include how to recognise, as well as control or
minimise the hazards associated with the lifting operation being performed or the lifting
equipment being used.
Training for the banksman/signal person shall specify the type of signalling (hand signal,
radio signal, etc.) the banksman/signal person is qualified to perform while training for
lifting equipment operators shall be type specific. As a minimum, all trainings shall be
tailored to meet applicable local regulatory requirements.

Personnel shall be re-trained if:
•

it is identified that they lack the skills/understanding needed to undertake safe
lifting operations
• there are changes at the worksite which presents additional hazards previously
unknown to personnel

• there are changes to the type of lifting equipment, fall protection, dropped object
protection or other equipment which presents a hazard previously unknown to
personnel
4.5 Competence

All personnel involved in planning lifting operations, performing lifting operations or
testing and maintaining lifting equipment, shall be trained and competent for their role.
A person shall be deemed competent when they have received the necessary training
and have adequate practical knowledge and experience of the lifts being undertaken.

Arrangements shall be in place to ensure that all personnel involved in lifting activities
undergo periodic assessments, as well as re-fresher training (as appropriate), in order to
maintain their competency.
All personnel involved with lifting activities shall be assessed for physical and medical
nc
fitness before being assigned duties in accordance with the Fitness for Work Standard
(PL-STAN-HSSEIA-HE-002).

4.6 Equipment
4.6.1 Requirements
All lifting equipment shall be designed to the appropriate equipment standard and
prevailing legislative requirements.
All lifting equipment shall be in accordance with the technical requirements for
certification, storage, maintenance and inspection criteria as specified by a recognised
industry standard and/or the manufacturer’s recommendations.
All lifting equipment shall be marked with the safe working load or working load limit and
be appropriately colour coded.
All lifting equipment shall be marked with a unique identification number. A certificate or

letter of conformance to the appropriate standard shall be issued by the manufacturer of
the lifting equipment or by an approved body.

An equipment register, including maintenance records and evidence of certification for
all lifting equipment, shall be kept and made available on site.

All lifting equipment shall be supplied with documentation which defines the:
• permissible operating conditions

design criteria

documentation for testing and maintenance

• examination/inspection requirements
• certificate of conformity issued by the equipment manufacturer

All lifting equipment shall undergo the required inspections by a competent person.
These inspections shall be completed at least once every 12 months and at least once
every 6 months for equipment used to lift people.
An inspection and maintenance schedule shall be established for all lifting equipment.
The schedule shall be based on the manufacturer’s recommendations, Petrofac
experience, operating environment, effects of aging and equipment utilisation, applicable
laws and failure modes of the equipment.
A process shall exist for the identification and removal of defective equipment from
service. Defective equipment shall be immediately tagged “Defective - Do Not Use” and
removed from service and quarantined.

Lifting equipment shall be withdrawn from use, repaired if necessary and re-certified if it
has been involved in any of the following:

• Any incident that may affect its integrity
• damage/changes or modifications to any load-bearing part
• inoperable safety devices

• changes in the condition of use, including periods out of service
Pre-use and post-use checks (visual and functional checks) shall be completed on all
lifting equipment and records shall be kept of all such inspections and maintenance.

4.8 Load integrity and stability

Lifting equipment operators and personnel involved in lifting operations shall be aware of
the limitations that can affect the lifting operation including the safe lifting capacity of the
equipment.

All the necessary information regarding the lifting equipment and the lifting operation
shall be provided to the personnel involved in an easily understandable format at the
place where the lifting equipment is being operated.
The Safe Working Load (SWL) of the equipment shall not be exceeded. The SWL shall
on
swing or fall unintentionally during the lift. Outriggers/stabilisers shall be used when
required and in accordance with the load chart or the lifting equipment manufacturer’s
instructions.
All personnel and third-parties shall be kept out of any area where they might be struck
U
or crushed by a load or lifting equipment if it swings, shifts or falls. No one shall stand or
work directly below a load.
The quality, design and labelling of packaging shall be specified at the procurement
stage to ensure load integrity and stability. Cargo in containers shall be secured in such
a way to prevent movement during the lift. Wheeled equipment shall be fitted with
suitable restraints to ensure stability.
Protection from dropped/falling objects shall be provided especially where people require
access to the area or where plant/equipment is at risk from impact. Loads shall be
adequately secured and properly inspected with any loose material which might
constitute potential dropped object removed before the load is lifted. Hard hats shall be
worn at all times.
A lift Drop Zone shall be established for each lifting activity. The trajectory (path) of the
lifted load shall be considered whilst establishing the lift Drop Zone. The
slinger/banksman shall ensure that the load is marshalled along its trajectory ensuring

that no one is positioned under the lifted load.
Appropriate barriers, notices and signage shall be used to prevent unauthorised entry

into a lift Drop Zone. Notices and warning signage shall be written both in English and in
the predominant local language.

As part of incident investigation, DROPS calculator shall be used to determine the
potential consequence severity of a dropped object as outlined in the Incident Reporting,
Classification, Investigation and Analysis Standard (PL-STAN-HSSEIA-011).

Lifting accessories and lifting equipment used for lifting people shall not be used for any
other purpose. All connections between the lifting appliance and the personnel transfer
carrier or work basket shall ensure they cannot self-release under any circumstances.

The lift plan shall clearly specify the limits for personnel lifts and this shall indicate clearly
where the limit differs from that of other lifts. Immediately before the lift, the prevailing
weather conditions shall be checked against the weather limitations stated in the lift plan.
If there is a deviation between the prevailing weather conditions and the weather
limitations stated in the lift plan, the lift shall be stopped and risk assessment reviewed.

Equipment for lifting personnel shall be fitted with distinct safety mechanisms for

preventing the load from free-falling.
A rescue plan shall be prepared and included as part of the lift plan for all personnel lifts.
All equipment required to implement the rescue plan shall be readily available before
and during the lift. The rescue plan shall be practised at regular intervals and
