does not fit with your work experience it is fine to leave it out. This evaluation form may be used to
guide your internship site visit as well as to aid in conversation regarding your progress with your
supervisor. Keep your learning objectives in mind while completing this form. Please submit the

you are contributing in a meaningful manner to the goals of the organization?
