SUBJECT : Spot report: Accidental firing at Barangay Matuber,DBS

Maguindanao dated July 15, 2017 at around 11:45 AM

DATE : July 15, 2017

1.) Reference: Blotter entry no. 104Accidental firing transpired at

BrgyMatuber DBS Maguindano, dated July 15,2017

2.) OOA 2:30 PM of July 15, 2017 a cellphone call from PSI Abdulah
Commanding Officer of 1stManeuver Platoon PPSC-Maguindanao receive by this
office, to report an accidental firing incident of his men transpired at Barangay
Matuber DBS Maguindnano dated OOA 11:45 AM of July 15, 2017. Then after the
call immediately COP of this office order PO3 Solaiman to proceed to CRMC
hospital to verify the incident and to investigate. Action taken: OOA 2:55 PM of July
15, 2017 the investigator arrived at hospital and the investigation was preceding.
According to PO1 Zaidamen A.Bantilan (suspect) he was picked up his issued
pistol that fell into the sand and when he cleaned it, he took out the magazine and
checked it, if it is loaded or not but accidentally fired it and he hit his left hand and
also hit his companion PO1SalaidenBaraguir (victim) at his back.Then they brought
his companion to the hospital for medical treatment.

3.) For info: The disposition of the suspect and the firearm used
with the serial number of PNP 37324CAL. 9mm was under the custody of PSI
ABDULAH.
