I, Gerlie Mae S. Dimaano, Administrative Assistant II of Fortuna National High School, Socorro District,
hereby expressing my sincerest intention to transfer in Victoria District for the following reasons to wit that:

1. I undergone dilatation and curettage twice due to miscarriage last March 2020 and June 2021.
2. I have been working at Fortuna National High School since October 2016 up to present. With this
span of service, I have been also travelling this long. Considering the distance and climate, it is risky
on my part.
3. I am a bonafide resident of Leido, Victoria, Oriental Mindoro and I am planning to live there
permanently with my husband which could be of great help for my health condition. This is also a
great opportunity to extend my service to my hometown.
4. I want to spend more time with my family whenever they need my urgent attention since my father
is a senior citizen and my mother will be turning 60 but the long distance hinders us. There are no
other members of the family who will supervise and take care of them.

It is my fervent hope and prayer that this humble request of transfer will be given meritorious favor from your
very good office. Thank you for considering my letter of intent.

I, John-John C. Dimaano, Teacher I at Fortuna National High School, Socorro District, heartily applying for
transfer of teaching station in Victoria District for the following reasons to wit:

1. I have been teaching at Fortuna National High School since May 2018 up to present with utmost
dedication. With the span of service, I have been also travelling this long. Considering the distance
and the climate, it is very risky and tedious in my part to travel every day.
2. I will be joining my wife who also seeks transfer of station to Victoria District because we are
planning to live permanently in Leido, Victoria. It is also more convenient on my part if I will be
working here together with my wife since my wife undergone dilatation and curettage twice due to
miscarriage last March 2020 and June 2021.
3. It would be very accessible for me to grow professionally by pursuing my Master’s Degree.

In view of the above-mentioned reasons, I hope and I pray for your favorable consideration on this matter.
