If so, please tell me how and when you became a


Christian?
I always grew up as one, THEN realized when i was like 22
that I really never asked God into my heart and so i did.

Are you now actively involved in Church? Yes


If yes, how? come to church and help wherever
What church? dads
I honestly need to probably go somewhere else, but just
wanted to support him

Are there any prescription medications currently taking


for any medical issues?
Yes, I take effexorr
Do you have a medical doctor that you see regularly? No
Hes my best guy friend, so i can text him for anything
When was your last check up and/or blood work?
Hmmm within last year

What is the problem for which you are seeking help, as


YOU see it?
I do not want to feel like i need these pills to function and
get things done. I dont want to feel like I need them to feel
certain ways or to numb out
I want to not feel like i need a fix or sex

What expectations/hopes do you have regarding any


counseling?
to be free, to have keys and understanding to walk thru
this and be delivered

I just need to know for sure if you are you willing to put in
the effort to change? It will be hard, but so worth it.

Yes I am , I will be honest I am so nervous and scared


some... Its like a voice is saying you dont need this, you are
fine you are in control... but I know its a lie
